42例主干静脉损伤的诊治体会

目的探讨主干静脉损伤的诊断与治疗特点。方法本组42例主干静脉损伤:行静脉简单修补术11例,补片修补4例,静脉端端吻合术10例,自体静脉移植术10例,人工血管移植术7例,并对术后进行随访。结果36例病人痊愈出院,4例术中死于大出血,1例术后6h死于不可逆休克,1例髂静脉损伤者术后1月开始出现下肢肿胀反复发作。结论主干静脉损伤术前急救,准确的诊断,术中及时显露损伤静脉,控制大出血,正确的修复方法是影响治疗效果的关键。

Objective To study diagnosis and treatment of main vein injury. Methods Among 42 cases, 11 cases underwent single vein repaired, 4 cases synthetic material repaired, 10 cases end-end anastomosis, 10 cases vein transplantation, and 7 cases synthetic graft. All cases were followed up. Results 4 cases died in operation,1 case died in 6 hours postoperatively, and 36 cases fully recovered. Conclusion It is important for surgical treatment of main vein injury that prompt diagnosis, preoperative emergent resuscitation, bleeding control and correct repair method.
