高血压左心室肥厚及构型与脉压的关系

THE RELATION BETWEEN HYPERTENSIVE LEFT VENTRICULAR HYPERTROPHY WITH ITS PATTERNS AND PULSE PRESSURE

目的 探讨高血压患者左心室肥厚及构型与脉压的关系。方法 回顾性分析171例有或无左心室肥厚(LVH)的高血压病病人的脉压与左心室肥厚及构型的相关性。结果 ①高血压病患者LVH的程度与其脉压的相关系数为r =0 . 5 9(P <0 .0 5 ) ,表明LVH程度与其脉压间有较密切的关系。②三型肥厚组的脉压与无肥厚组相比,有明显的差别,对称性室间隔肥厚(CH)和扩张性肥厚(DH)较大(P均<0 . 0 1) ,尤以扩张性肥厚(DH)较甚。结论 高血压患者左心室肥厚及构型与脉压具有一定的联系,因此,对于肥厚程度较重、扩张性肥厚(DH) ,要给予高度的重视;在降压治疗的同时,应缩小增大的脉压。

Objective To investigate the relationship between hypertensive left ventricular hypertrophy with its patterns and pulse pressure (PP)in hypertension patients. Methods Parameters of blood pressure, clinical features and echocardiographic findings were analyzed retrospectively in 171 patients with essential hypertension. Results ①The degree of LVH and pulse pressure (PP) grade has close relationship (r=0.59, P<0.05). ②three types of hypertrophy have a positive correlation with age, duration of hypertention and pulse pressure. And the dilated hypertrophy has the higher pulse pressure (P<0.01). Conclusion The pulse pressure has a positive correlation with hypertensive left ventricular hypertrophy or patterns of hypertrophy. Hypertensive patients with severe LVH and asymmetric LVH should be attached great importance. In order to decrease the cardiac damages of hypertension during lower blood pressure treatment, more attentions should be put on pulse pressure.
