食管-气管导管快速建立人工气道及通气效果观察

摘要 目的 评估食管气管联合导管(ETC)快速建立气道后的通气功能。方法 乳腺癌病人30 例随机分为ETC组(C组,n=15)和气管导管组(T组,n=15)。诱导后喉镜明视下C组食管插入ETC,T组经声门插入气管导管。麻醉中呼吸参数设定相同。监测围术期的血流动力学,通气功能及气道力学的变化。结果 插管前两组间各种参数无显著差异。两组间插管所用时间无显著差异。术中两组间SBp、DBp、HR、SpO2、呼末二氧化碳(ETCO2)无显著差异(P>0.05),但C 组吸气峰压明显高于T组(P<0.05);一秒率和肺顺应性明显降低且与T组比较有显著差异(P<0.05)。结论 ETC在心肺复苏急救时能快速建立有效的人工气道。 Objective To evaluate the ventilation functions of the airway quickly established by esophagesl-tracheal Combitube (ETC).Methods Thirty patients with breast cance were randomly divided into two groups:group C(Combitube, n=15) and group T (Tracheal tube, n=15) .The Combitube were intubated into esophageal of patient after induction in group C and tracheal tube in group T. Hemodynamic and ventilation function parameters were monitored ( Datex-Ohmeda S/5 monitor).Results There were no significant differences in the parameter between the two groups before induction.There was no difference in time to induction between the two groups . After Combitube intubation in group C of 35 min ,Lung compliance (C L )and first second forced expiratory flow rate (FEV 1.0% ) were decreased significantly and peak pressure (Ppeak) was increased significantly.There was significantly difference between two groups (P< 0.05 ).Marked changes in SBp,DBp,HR,SpO 2 and ETCO 2 were not found between the two groups during operation (P> 0.05 ).Conclusion Using Combitube can establish an effective airway for CPR.
