血清C-反应蛋白对不稳定型心绞痛患者预后的预测价值

Prognostic Anticipant Value of Blood Serum C-Reactive Protein on Unstable Angina Pectoris Sufferers.

目的 探讨不稳定型心绞痛(UAP)患者血清C -反应蛋白(CRP)水平与心血管事件发生的关系。方法 将UAP患者按入院2 4小时内血清CRP水平分为两组:低CRP组(CRP <3 .5mg·L-1) 4 6例,高CRP组(CRP≥3. 5mg·L-1) 5 8例。两组患者入院后均予常规内科治疗,比较2周内及随访6个月时两组发生心血管事件的情况。结果 (1)低CRP组2周内发生心绞痛总数次15 8例次,平均3 .4 3次/人,与高CRP组总2 86例次,平均4 .93次/人比较有显著性差异(P <0 . 0 5 ) ;随访6个月时,低CRP组共发生心绞痛2 0 1例次,平均4 .37次/人,与高CRP组总76 3例次,平均13. 1次/人比较有显著性差异(P <0 . 0 1) ;(2 )低CRP组2周内及随访6个月时发生急性心肌梗死(AMI)例数(% )分别为6例(13 .0 % )及2例(4 .4 % )与高CRP组18例(31 .0 % )及13例(2 2. 4 % )比较有显著性差异(P <0 . 0 5 ) ;(3)低CRP组随访6个月时发生猝死1例(2 . 2 % )与高CRP组5例(8. 6 % )比较无显著性差异(P >0 . 0 5 )。结论 血清CRP水平对UAP患者近远期心血管事件的发生有很高的预测价值。

Objective Explore the relationship between C-reac ti ve protein (CRP) of unstable angina pectoris (UAP) sufferers and cardiovascular disease. Methods According to the CRP level after entering hospi tal 24 hours, 104 patients with UAP were divided into 2 groups: low CRP group (C RP<3.5mg/L; 46 cases) and high CRP group (CRP3.5mg/L;58 cases). All patien ts in two groups were given routine internal medicine treatment. Compare the inc idence of cardiovascular disease in 2 weeks and in following 6 months. R esults (1)There were 158 and 286 UAP occurring in low CRP group and high CRP group, respectively (average 3.43 times/person and 4.93 times/person, respe ctively) in 2 weeks. UAP incidence in high CRP group was significantly higher th an that in low CRP group (P<0.05). In the following 6 months, there were 2 01 and 763 UAP occurring in low CRP group and high CRP group, respectively(avera ge 4.37 times/person and 13.1 times/person, respectively). UAP incidence in hig h CRP group was markedly higher than that in low CRP group (P<0.01). (2)In 2 weeks and 6 months, there were 6 and 2 acute myocardial infarction (AMI) in lo w CRP group and 18 and 13 AMI in high CRP group. AMI incidence in high CRP group was markedly higher than that in low CRP group (P<0.05). (3)Sudden death in high CRP group and low CRP group was no difference (P>0.05). Co nclusions CRP has highly anticipant value for UAP and AMI.
