保留十二指肠胰头部肿瘤切除术(Beger改良术)

Reformative Beger's Operation

目的探讨符合解剖、生理学要求的胰头部肿瘤的新术式。方法本术式所做的保留的十二指肠胰头部切除,包括肿瘤在内的胰头部一并切除,并采用保留的十二指肠和残胰、胆管原位重建。结果2例手术均顺利进行,手术时间分别为2.5h和3.5h,均在术后半个月临床治愈出院。结论保留十二指肠胰头部肿瘤切除术,能保留消化道解剖、生理学上的正常状态和完整性,有较大优点,但手术操作有一定难度。

Objective To explore a new operative way to the cancer in the head of pancreas that accords with anatomy and physics on the basis of Beger's operation.Methods The head of pancreas including the cancer was wholly removed, and the conserved duodenum, remained pancreas and bile duct were reconstructed in situ. Results The operations in two patients were performed smoothly. The operative durations were respectively 2.5 and 3.5 hours. They both achieved clinical cure and left the hospital in half a month. Conclusion Reformative Beger's operation has its strongpoint in conserving the normal digestive anatomy and physics in spite of difficulty to some extent.
