改良Madigan前列腺切除术82例报告

A Report of 82 Cases of Improved Madigan's Prostatectomy

目的探讨提高前列腺切除术的治疗效果。方法应用改良的Madigan前列腺切除术治疗前列腺增生症82例。结果手术时间平均51min,术中失血量平均230ml,62例术中未输血,68例术后未行膀胱冲洗,尿管留置平均5.5d,术后无继发性出血及尿失禁。出院时IPSS评分平均6.2分,QOL平均1.8分。结论改良的Madigan前列腺切除术,保留了完整的尿道和膀胱颈黏膜,并同时处理膀胱内病变,手术疗效好,术后恢复快,并发症少,疗效满意。

Objective To evaluate the treatment effect of prostatectomy. Methods Modified Modigan's prostatectomy was employed to treat benign prostatic hypertrophy in 82 cases.Results The average operating time was 51 minutes, and the average blood loss was 230ml. There were 62 cases that experienced no blood transfusion in the operation. No bladder wash was performed in 68 cases after operations. The catheter was retained at an average of 5.5 days. There was no secondary bleeding or urinary incontinence. The average IPSS score was 6.2 and the average QOL was 1.8 when they left the hospital. Conclusion The modified Madigan's prostatectomy preserves the integrity of the ureter and mucosa of the bladder neck, and its management of the lesions in the bladder cavity produces satisfactory results with less complications and rapid recovery.
