A population-based study of the optic disc size

目的 评价并比较4 0岁以上人群中正常人及青光眼患者的视盘大小。设计 以人群为基础的横断面调查研究。研究对象 源于北京地区人群为基础的眼病调查,评价4 0岁以上受检者清晰的眼底照片,其中正常人3918例,原发性青光眼135例。方法 被调查者均接受视力、裂隙灯、眼底照像等眼科检查。应用独立样本t检验比较正常人及青光眼两组视盘的大小。主要指标 测量视盘的垂直径和水平径,计算视盘平均直径和视盘面积。结果 正常人视盘的垂直径为(1 97±0 19)mm ,水平径为(1 78±0 18)mm ,平均直径为(1 87±0 17)mm ,视盘面积为(2 77±0 5 1)mm2 (1 4 2～6 4 2mm2 )。小视盘平均直径≤1 6 1mm ,大视盘平均直径≥2 15mm ,中视盘平均直径介于二者之间。青光眼视盘垂直径为(1 93±0 2 0 )mm ,水平径为(1 83±0 2 4 )mm ,平均直径为(1 88±0 2 0 )mm ,视盘面积为(2 80±0 5 9)mm2 (1 6 3～5 0 3mm2 )。正常人与青光眼患者的视盘平均直径(P =0 6 7)、视盘面积(P =0 5 2 )均无显著性差异。结论 人群中视盘大小变异较大,正常人与青光眼患者的视盘大小无明显差异。

Objective To evaluate the optic disc size in population aged 40 years or more,compare the optic disc size between normal subjects and glaucoma patients.Design Population-based cross-sectional study.Participants All participants aged 40 years or more were selected from the database of Beijing Eye Study.Good quality fundus photographs for 3918 normal subjects and 135 primary glaucoma patients were evaluated.Methods All participants had undergone ophthalmologic examinations including visual acuity,slit-lamp biomicroscopy and fundus photography.Optic disc photographs were measured.Independent-samples T test was used to compare the disc sizes between normal subjects and glaucoma patients.Main Outcome Measures Horizontal and vertical disc diameters,mean values of the vertical diameter and horizontal diameter,and optic disc area.Results The vertical disc diameter in normal population was 1.97±0.19mm,the horizontal disc diameter was 1.78±0.18mm.The mean value of vertical and horizontal diameter was 1.87±0.17mm in normal population.Optic disc area was 2.77±0.51mm 2 (1.42-6.42mm 2).Percentiles 5 and 95 in the frequency distribution of optic disc diameter were defined as cutoff for large and small size discs,the middle of 90% for medium size disc,that was,disc diameter was less than or equal to 1.61mm for small disc,disc diameter was more than or equal to 2.15mm for large disc,the disc diameter ranged between 1.61mm to 2.15mm for medium disc.In glaucoma patients,the vertical disc diameter was 1.93±0.20mm and the horizontal one was 1.83±0.24mm,thus,the mean value of vertical and horizontal diameter was 1.88±0.20mm.Optic disc area in glaucoma patient was 2.80±0.59mm 2 (1.63-5.03mm 2).There was no statistically difference in the optic disc diameter and disc area between normal subjects and glaucoma patients (P=0.67,P=0.52).Conclusions The size of optic disc in the population shows significantly variable.The optic disc size between normal subjects and glaucoma patients demonstrates no significant difference.
