输尿管肾镜下气压弹道碎石术治疗输尿管结石(附328例报告)

Treatment of ureteral calculi by pneumatic lithotripsy under ureterorenoscope

目的探讨输尿管肾镜下气压弹道碎石术治疗输尿管结石的临床应用价值。方法回顾性分析328例经输尿管肾镜下气压弹道碎石术治疗的输尿管结石患者的资料。结果328例输尿管结石中,298例1次碎石成功,成功率91%,9例进镜困难(其中5例致输尿管穿孔),改开放手术。随访1～3月,平均1.5月,3个月内结石均排净。结论此治疗方法安全、高效、损伤小、恢复快,可作为输尿管结石的首选治疗方法。

Objective To evaluate the clinical effect of pneumatic lithotripsy under ureterorenoscope in the treatment of ureteral calculi.Methods Three hundred and twenty_eight cases of ureteral calculi were treated by ureterorenoscopic pneumtic lithotripsy from May 2000 to Marth 2004.Results lithotripsy was performed successfully on one session in 298 cases with a success rate of 91%(198/328).conversions to open surgery were required in the remaining 9 cases because of unsuccessful entry of the ureteronoscope (ureteral perforation in 5 cases).Follow-up period ranged from 1 to 3 months (mean,1.5 months).Stones were entirely expelled within 3 months after the procedures.Conclusions With minimal invasion and pain,this procedure is safe and effective for ureteral calculi,being probably the treatment of first choice.
