摘要
目的 总结和分析比较AF系统和RF系统治疗胸腰椎骨折的复位原理及疗效。方法 对85例胸腰椎骨折患者随机分为两组, AF组45例, RF组40例, 经AF系统或RF系统固定加后路植骨治疗胸腰椎骨折, 并进行随访分析, 观察骨折椎体高度的变化情况, 脊椎cobb角和椎管面积的改变以及神经功能恢复程度。结果 伤椎cobb角AF组由术前的32 6度恢复到术后2 8度, RF组由术前的30 5度恢复到术后3 4度, 两组比较差异无显著性意义, 伤椎高度恢复良好, 形态无塌陷, 伤椎管面积AF组由术前的46 1%恢复到术后81 7%, RF组cobb角由术前的44 2%恢复到术后84 5%, 两组比较差异无显著性意义, 术后两组神经功能获得Frankel1 -3级改善。结论 AF系统和RF系统是治疗胸腰椎骨折良好的内固定器械, 两者复位原理不同, 但疗效基本相同, AF系统较RF系统结构简单, 操作简便, 节约了手术时间。
Objective To summarize and analysis the diaplasis principle and effect on treatment of thoracolumbar fracture with AF device fixation and RF device fixation.Methods Eighty-five cases of thoracolumbar fracture were treated two groups ,45(AF) and 40(RF).AF device fixation or RF device fixation and graft bone were used to cure thoracolumbar,and observed the variance of the height of the fracture verbebral body ,spinal cobb angles , the chang of spinal canal area and the degree of nerve's function.Results The cobb angles of fractured verberbral body of AF group's was decreased from 32.6 degree to 2.8 degree,RF groups was from 30.5 degree to 3.4degree,there were no significant different. The height of the fractured veterbral body recover well,the area of spinal canal AF groups was recovered from 46.1% to 81.7%,spinal cobb angles of RF groups' was recovered from 44.2% to 84.5.%The two groups cases had improved nervous function by one to three grades according to Frankels grading.Conclusion AF device fixation and RF device fixation were two of effective,convient,security procedures for treatment of thoracolumbar fracture.Although they had different reduction principles ,they had identical treatment effect ,and AF device fixation was simpler ,and more concient than RF device fixation.
