cITP患者糖皮质激素治疗前后Th1/Th2改变 被引量:3

Changes of Th1 and Th2 before treatment and after treatment of corticoid in patients with chronic idiopathic thrombocytopenic purpura

摘要 目的 探讨慢性特发性血小板减少性紫癜(cITP)患者Th1/Th2功能极化改变和糖皮质激素治疗效果之间关系。方法 采用流式细胞仪对18例cITP患者治疗前后和18 例健康者外周血CD3+、CD8- 细胞内IFN-γ和IL-4进行分析。结果 cITP患者治疗前Th0、Th1细胞和Th1/Th2比值较对照组显著降低(P<0 01),Th2细胞升高(P<0 05),糖皮质激素治疗有效者治疗后Th0、Th1 细胞和Th1/Th2 比值较治疗前显著增高(P<0 01),Th2细胞无明显差别(P>0 05),治疗无效者Th0、Th1、Th2 细胞和Th1/Th2 比值无明显差别(P>0 05)。结论 cITP表现为Th2优势表达,糖皮质激素治疗cITP主要通过上调Th1 细胞水平,纠正Th1/Th2 比值异常达到治疗目的。 Objective To study the relationship between changes of Th1/Th2 polarization in chronic idiopathic thrombocytopenic purpura(cITP) and therapy of corticoid.Methods 16 patients of cITP and 18 health contrlos were detected for intracellular factors(IFN-γ、IL-4) in CD3+、CD8- cells by flow cytometry before and after the treatment.Results In the patients before treatment,Th0,Th1 and the Th1/Th2 ratio were significantly decreased in cITP (P<0 01),Th2 was higher in cITP compare with health controls (P<0 05), in the patients after treatment,Th0,Th1 significantly increased (P<0 01),Th2 cells was no difference compared with the data before treatment (P>0 05). Conclusion Our results suggest that cITP may reflect Th2 cells activation,the effect of treatment with corticoid may tightly related with the increase of Th1 cells and the improvement of Th1/Th2.
