显微斜视手术后眼屈光的动态变化

Dynamic refractive changes in eye after strabismus surgery

目的 探讨显微斜视手术后短期内眼屈光改变的趋势。方法 6 7例水平斜视患者,有3例为双眼手术,其余均单眼手术。手术眼70只眼,非手术眼6 4只眼。非手术眼为对照组。12岁以下儿童用美多丽扩瞳,5分钟1次,共6次,30分钟后验光。其余直接验光。术前1次,术后第1、2、3、4周各1次。每次的数据是3次比较稳定的数据平均而得。在5次检查中都做了矫正视力检查。术后第1、2、3、4周的屈光度数与术前的度数作比较,球镜与柱镜分别比较。结果 对照组术后各周屈光与术前比较球镜和柱镜都没有显著差异,不同时间均值接近,差别很小。各周之间P >0 .0 5。手术组球镜不同时间的平均值非常接近,差别没有显著意义。P值均>0 .0 5。而手术组的柱镜不同时间有显著性差异。散光基本在水平方向上。并随着时间的推移向术前回归。与术前比较,术后第1周差别最大,以后变小,第4周最小。所有病例第4周的矫正视力均不低于术前的矫正视力。结论 斜视手术后眼屈光改变在被手术肌肉所在的经线上,表现为近视散光加深,随时间推移逐步回归到术前水平,在术后第4周时基本稳定。

Objective To investgate refractive changes in eyes after strabismus surgery .Method The eye refraction of 67 patients (70 eyes)were examined before operation and 1,2,3,4 weeks after operation.The other un-sergery eyes were control group.Refraction of children under 12 were finished after 6 times Ttropicamide-Phenylephrine Eyedrops .The 4 post-operative refractive values were compared with pre-operative refractive value.Result In control group,there were no significantly difference in sphere and astigmatism degree.In surgery group no significant difference was found in sphere degree,but significant difference were found in astigmatic degree.The big astigmastic value presented 1 week postoperation,then decreased to preoperation value 3-4 weeks postoperation.The best corrected visual acuity of the 4 weeks postoperation was eqeal to or better than that of preoperation. Conclusion Astigmatism occurred due to strabismus surgery,and decreased over time to presurgery value at3- 4 weeks after surgery.
