下咽及喉部疾病所致阻塞性睡眠呼吸暂停低通气综合征8例

Hypopharyngeal and laryngeal diseases as a cause of obstructive sleep apnea hypopnea syndrome

目的：提高临床医师对阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome，OSAHS)少见致病因素的认识。方法：回顾性分析我科收治的8例下咽及喉部疾病所致OSAHS的临床表现、诊断及治疗方法。结果：8例下咽及喉部疾患均首先以OSAHS就诊；全部病例均采用相应的手术治疗，其中2例成人后天特发性喉软骨软化症及3例巨大会厌囊肿术后OSAHS症状完全缓解，随访0．5～11a无复发；1例下咽滑膜肉瘤及2例会厌鳞状细胞癌．随访5～7a肿瘤无复发。结论：OSAHS可由下咽及喉部疾患引起，临床应予以重视。

Objective:To enhance the recognition of hypopharyngeal and laryngeal diseases as a cause of obstructive sleep apnea hypopnea syndrome(OSAHS).Methods:The clinical presentation,diagnosis and management of 8 cases with hypopharyngeral and laryngeral diseases were reviewed retrospectively between Jun.1993 and Mar.2004.Results:All of the 8 cases were presented with OSAHS in the first visit and treated by surgery.The cure rate was 100% with a follow up duration from 0.5 to 11 years.Conclusion:The OSAHS may be caused by hypopharyngeal and laryngeal diseases.Patients with OSAHS need to be evaluated with laryngoscopy.We should pay more attention to this condition and avoid improper treatment.
