小剂量肿瘤坏死因子对感染性休克的保护作用

摘要 本文观察了预先给予大鼠小剂量肿瘤坏死因子(tumornecrosisfactor,TNF)对感染性休克的作用。结果发现,给大鼠腹腔注射5μg重组人TNF或1.2×10 ̄5U肿瘤坏死血清,16h后再给予致死量活大肠杆菌及其灭活冻融死菌时,与单纯感染性休克大鼠相比,平均动脉压下降程度轻,持续时间短;感染后4~6h脉压差缩小程度明显减轻,存活时间显著延长;感染后6h动脉血乳酸水平升高较少,血球比积无明显增大。本研究表明,预先给予小剂量TNF或肿瘤坏死血清,可以明显减轻和防止感染性休克的发生。 Male Sprague-Dawley rats(200~250g,n=48)were given i.p.1ml of TNS (1.2 × 10 ̄5 U)(group S)or TNF(5μg)(group F) or normal saline (group E)16 hbefore experimentation,then infused with E.coll O_111B_4 1×10 ̄(10) cfu/100g b.w. plus equalvolume of its boiled-deactivated frozen-thawed bacterial suspension. After infusion, bloodpressure(BP) in group E droped promptly to 5.73±2.33 kPa within 15 min and kept in 9.33 kFa for about 4h,but BP in group F and S were much higher than that ingroup E,which persisted in 13.33~16.0 kPaP<0.001~0.05). Pulse pressure in groupF or S was also higher than that of group E at 4h (P<0.01 ) and 6h(P<0.05)after E.coli infusion. The survival time in group F (12.0±3.4 h) or group S(12.3±3.0h)was significantly longer than that in group E(5.6±2.0h).If E. coli was infused in half quantity,the survival time in group E elongated to about 16h and group F to morethan 5 days.The hematocrit and concentration of arterial lactic acid 6h after infusion ingroup E were significantly increased than that in group F( P<0.05).The results indi-cated that small dose of TNF or TNS given prior to E. coli infusion may protect theanimal against lethal septic shock.
