人工股骨头置换术后疼痛原因分析

Analyze the Causes of the Pain after Artifical Femoral Head Replaced

目的探讨人工股骨头置换术后疼痛发生的原因及其处理方法。方法对81例人工股骨头置换术后的患者进行随访,随访时间1～15年,并对随访结果作统计学处理。结果81例中有31例存在不同程度的疼痛,随术后时间的延长,疼痛的发生率逐渐升高,其中6例有X线表现松动征象,11例有临床松动症状。4例异位骨化患者术后中度疼痛,2例晚期感染,术后严重疼痛。结论假体移位下沉是造成人工股骨头置换术后疼痛的主要原因:X线松动征象与疼痛关系并不确定,疼痛取决于临床松动程度;感染及异位骨化是造成术后早期疼痛的重要原因。

Objective To investgate the causes of pain after areificial femoral head replaced, and how to deal with it.Methods 81 cases were followed up for an average of 5 years.All of them were taken X-ray of pelvis,and the data were analyzed statisticaly.Results In 81 cases, 31 cases had pain to some extent ,and the rate of pain increased with the time extending .6cases with X-ray loosening and 11cases with clinical loosening had pain.4 cases with heter otopic ossification had moderate pain and 2 cases with terminal infectiopon had serious pain.Conclusion The main cause of pain after artificial femoral head replcement is the loosening or subsidence of the prosthesis .The relationship between the pain and the X-ray loosening is not certain.The pain lies on the degree of loosening .The infection and heterotopic ossification are important reasons of early pain after operation.
