摘要
目的:对广西壮族自治区进行抽样调查,探讨该地区正常人群肺功能的生理预计值及其不同生活环境、年龄、身高、体重等因素与肺功能的关系.方法:2004年9月对进行过基础体检并确定为正常的 1 056 名不同年龄段人群,采用便携式肺功能仪进行肺容量及肺通气量检测,分析不同地域、年龄、性别、身高、体重等因素对肺功能的影响.结果:检测的各项肺功能指标城乡之间差异有显著性(P<0.05);多数肺功能指标(男性9项,女性10项)城市大于乡村(P<0.05);不同性别之间差异有显著性(P<0.05).年龄段的比较12~18岁肺功能增长迅速,每2岁一个年龄段的变化大于或等于成年人10岁一个年龄段的变化,其中补呼气量(ERV)、深吸气量(IC)、肺活量(VC)、用力呼气肺活量(FVC)18岁达到高峰;最高呼气流速(PEF)、最大通气量(MBC)26岁达到高峰,各项指标达高峰值后能持续该峰值至34~44岁,然后呈逐渐下降趋势(P<0.05),呼气流量指标:25% 呼气流速(FEF25%)、FEF50%、FEF75%、中段呼气流速(MMEF75/25)18岁达高峰后即开始下降(P<0.05).年龄、身高、体重与肺活量(VC)增长均有相关性(P<0.05),其中身高的影响最大,年龄次之,体重对成年男性(19~77岁)及45岁以上女性无影响.结论:肺功能的生理预计值存在着地域差别,不同民族及生活水平的差异可能是因素之一,肺功能随年龄增长而发生变化,少儿期、成年期、中老年期应有不同的生理预计值;本研究为正常人群肺功能提供了参考值.
Objective:To put up the spot test and investigate the lung function of the normal subjects living in the Zhuang autonomous regions , Nangning city and Binyang county in Guangxi province and its correlative factors such as living circumstance, age, height, and body weight.Methods:The lung volumes and breath capacities of 1 056 normal subjects were tested by portable spirometers (Scope Rotry ) in September 2004.Results:Significant difference existed between urban and rural areas in all indexes ( P <0.05); there were nine indexes for male subjects and ten for female subjects; The value of urban indexes was more than rural( P <0.05).Significant difference existed between genders ( P <0.05). Growth spurt appeared at the age of 12~18, its change in every two-year-phase was more obvious than or equal to that in ten-year-phase of grown-up. Expiratory reserve volume,(ERV), inspiratory capacity,(IC),forced expiratory volume per second (FEV1),vital capacity (VC), forced vital capacity (FVC), Reached their peaks at the age of 18.Peak expiratory flow (PEF), maximal breathing capacity(MBC), reached their peaks at the age of 26 and maintained these levels between 34~44, then decreased gradually by age average ( P <0.05). The curve of expiratory flow: 25% forced expiratory flow (FEF25%)、FEF25%、FEF25% and maximal mid-expiratory flow(MMEF75/25)reached their peaks at the age of 18 and then declined ( P <0.05). Age, height and weight were correlative with increase of vital capacity (VC) ( P <0.05). Among these factors, height had the most influential effect while age took the second place. Weight had no impact on male between 19~77 and female above 45.Conclusion:Expected values of lung function were changed regionally probably because of different nationalities and living conditions. Lung function changes with age, therefore different expected values shall be available for the adolescence, young adults, middle-aged and old people. This study provides reference values of lung function.
