经尿道前列腺等离子双极汽化电切术(附25例报告)

摘要 目的:探讨经尿道前列腺等离子双极汽化电切术的疗效和安全性。方法:采用英国Gyrus公司提供的等离子体双极切割系统(Gyrus Plasma Kinetic System,PKS), 行经尿道前列腺切除术25 例。结果:25 例手术顺利,手术操作时间平均45min,平均切除前列腺38g, 无输血病例,术中无闭孔神经反射及电切综合征(TURS)出现。术后平均留置导尿管时间3.5 d,拔除尿管后排尿通畅,尿路刺激症状轻。术后随访3个月,无尿失禁、尿道狭窄等并发症。全部病例排尿通畅,前列腺症状评分从术前26.5降至5.8,平均最大尿流率从术前9.2ml/s增至21.5 ml/s。结论:经尿道前列腺等离子双极汽化电切术是一种安全有效的微创术式,尤其适合于高危、重度前列腺增生病人。 Objective: To investigate the efficacy and safety of transurethral plasma kinetic vaporization prostatectomy(TUPVP) in benign prostate hyperplasia by perplalia.Methods: 25 patients with bengin prostate hyperplasia (BPH) were treated by TUPVP using the Gyrus-Plasma Kinetic System(PKS).Results: All the 25 patients underwent the operation successfully. Mean operation time was 45min. Mean resected prostatic tissue was 38g, no case need blood transfusion during the operation, no aductor reflex and transurethral resection syndrom(TURS) occurred. The mean time of an indwelling urethral catheter was 3.5days, After pulling out the pipe, voiding freely. All patients were followed-up for 3 months,no complications as incontinence and urethral stricture were found. All patients were discharged successfully,I-PSS decreased from 26.5 to 5.8,Qmax increased from 9.2ml/s to 21.5ml/s.Conclusion:Transurethral plasma kinetic vaporization prostatectomy was a safe and effective microinvasive surgical method, especially for the patients in high risk and with large volume benign prostate hyperplasia.
