糖尿病视网膜病变与VEGF关系的研究新进展

摘要 糖尿病视网膜病变(diabetic retionopathy,DR)是糖尿病并发症中最为常见和严重的微血管病变之一,也是糖尿病病人致盲的重要原因之一[1,2].因此,DR的发病机制及其防治成为国内外眼科界的研究热点[3].DR的典型眼底病变可见视网膜微血管瘤、点片状渗出、新生血管形成、玻璃体出血,甚至视网膜脱离而致失明.研究发现[4],DR发生率随糖尿病病程发展有所不同.在全部糖尿病患者中有DR者,达50%以上,病程越长,发病率也越高.目前认为,DR的发病机制错综复杂,是多种因素协同作用的结果,各种因素相互依存、相互影响,共同参与DR的发生、发展.在DR发病机制中,受大多数学者认可的观点是"多元醇通路的激活、肌醇耗竭、蛋白的非酶性糖基化、自由基损伤及二酰基甘油一蛋白激酶C系统的活化"[3,5].近年研究[6],血管内皮生长因子、血小板生长因子、成纤维细胞生长因子、肿瘤坏死因子及生长激素等均与DR的发生、发展有密切的关系.其中,血管内皮生长因子(VEGF)特异性最强,在眼内新生血管形成中起关键作用[7].本文就VEGF及其与DR的关系作一综述.
