单侧成角度骨外固定架结合有限内固定治疗股骨转子下粉碎性骨折

Treatment of comminuted subtrochanteric fracture by combining unilateral angular external fixator with limited internal fixation

目的探讨单侧成角度骨外固定架结合有限内固定治疗股骨转子下粉碎性骨折的效果.方法对15例股骨转子下粉碎性骨折患者采用切开复位、钢丝和(或)螺丝钉内固定联合单侧成角度骨外固定架固定.结果随访8～24个月,所有病例均骨性愈合,平均愈合时间6个月.疗效评定:优12例,良2例,可1例.1例针道感染.无1例发生螺纹针断裂、松动及髋内翻等并发症.结论单侧成角度骨外固定架结合有限内固定治疗股骨转子下粉碎性骨折是一种有效可靠的方法,能达到良好的骨折复位,坚强的固定,并可早期行关节功能锻炼.

Objective To investigate the clinical efficacy of treatment of comminuted subtrochanteric fracture by combining unilateral angular external fixator(UAEF) with limited internal fixation. Methods 15 cases with comminuted subtrochanteric fracture were treated with open reduction and internal fixation with steel wire and screws combined with UAEF. Results 15 cases were followed up for 8～24 months. Complete union was found in all cases in 6 months on average. The results were excellent in 12 cases, good in 2 and fair in 1. No screw looseness, breakage or hip varas was found. 1 got pin tract infection. Conclusions It is an effective and reliable method to treat comminuted subtrochanteric fracture by combining UAEF with limited internal fixation. It has obvious advantages of good reduction and reliable fixation, which is useful for early joint function exercise.
