改良国产伽玛钉治疗不稳定型转子间骨折

Treatment of unstable intertrochanteric fracture with improved domestic gamma nails

目的探讨改良国产伽玛钉治疗不稳定型转子间骨折的临床效果.方法采用改良国产伽玛钉治疗不稳定型转子间骨折32例,按Evans-Jensen骨折分型ⅡA型8例、ⅡB型13例,Ⅲ型11例.结果平均随访20个月(6～36个月),32例骨折全部愈合,临床愈合时间平均为10.5周(8～24周).患肢功能按Sanders髋关节创伤评分标准,优良率93.8%(30/32).1例老年患者术后关节疼痛,1例髋内翻.结论用改良国产伽玛钉治疗不稳定型股骨转子间骨折愈合率高,并发症少,是目前较好的内固定方法.

Objective To report the clinical results in the treatment of unstable intertrochanteric fractures with improved domestic gamma nails. Methods 32 cases of unstable intertrochanteric fractures were treated with gamma nails. Among them 8 cases belonged to Evans-Jensen type ⅡA, 13 ⅡB and 11 Ⅲ. 11 cases had associated injuries. Results All cases were followed up for 20(6~36) months on average.The fractures all healed in the 32 cases. The clinical healing time averaged 10.5(8~24) weeks. The functional evaluation was done with Sanders traumatic hip rating scale. The excellent and good rate was 93.8%(30/32). One elderly patient experienced hip pain after surgery and one had hip varus deformity. Conclusions The gamma nail can be used to treat unstable intertrochanteric fractures with good results, high union rate and rare complications. It is by far a good fixation method.
