牵引成骨速率适应组织功能性变化的实验

摘要 目的:为减少牵引成骨过程中软组织变化对成骨效果的不良影响,探讨下颌骨牵引延长中二腹肌肌束膜对不同牵引速率的功能性变化。方法:实验于2003-12/2004-03在第四军医大学口腔医学院口腔颌面外科实验室完成。以1,2mm/d牵引速率双侧前徙兔下颌骨1.2cm,分别于牵引完成时、牵引完成后2,4周取二腹肌标本,石蜡切片,苏木精-伊红染色,天狼猩红苦味酸染色,偏光显微镜、photoshop软件分析肌束膜厚度及Ⅰ型、Ⅲ型胶原含量的变化。结果:1mm/d牵引速率下肌束膜损伤小,并功能性增厚,2mm/d牵引速率下肌束膜明显萎缩。结论:1mm/d的牵引速率是适宜的,更能适应组织的功能性变化。 AIM:To investigate the functional changes of the perimysium of digastric muscl e in response to different distraction rates in rabbit mandible in order to supp ress the bad influence of soft tissues on distraction osteogenesis. METHODS:The experiment was done in the labartory of Oral and Maxillofacial Sur gery,Stomatological Hospital,Fourth Military Medical University of Chinese PLA f rom December 2003 to March 2004.The mandible of rabbits was advanced bilaterally for about 1.2 cm at the rates of 1 and 2 mm per day respectively.The samples of digastic muscle were made post distraction osteogenesis,after a two week cons olidation and after a four week consolidation to make paraffin sections followi ng by hematoxylin and eosin staining.Picrosirius polarization method was used t o investigate the thickness of the perimysium and the proportional changes of ty pe Ⅰand Ⅱcollagen under a polarization microscope using Photoshop software. RESULTS:The perimysium became thicken at the distraction rate of 1 mm per day. When the mandible was distracted at the distraction rate of 2 mm per day,the per imysium became thinner obviously. CONCLUSION:The distraction rate of 1 mm per day is appropriate for the functio nal changes of intramuscular connective tissues.
