联合检测p53及p27和bcl-2对非小细胞肺癌早期诊断及其实施干预的价值(英文)

摘要 背景:肺癌患者因抑郁、肺功能降低、自觉体力下降、疲乏、精力不济等影响生活质量;如出现并发症或晚期恶病质将影响患者生存期。目的:研究p53,p27,bcl-2三种基因蛋白与非小细胞肺癌病理特征的关系。设计:以病理标本为研究对象的对照实验研究。单位:一所大学医院呼吸科和一所大学中心实验室。对象:1997-06/2002-12手术切除非小细胞肺癌组织标本76例,均为原发性肺癌,且未接受任何治疗方法应用SP免疫组化法检测76例手术切除非小细胞肺癌标本中三种基因蛋白的表达。主要观察指标:p53,p27,bcl-2的阳性表达情况。结果:76例标本中28例(37%,28/76)p53过度表达,34例(45%,34/76)p27过度表达,37例bcl-2过度表达(49%,37/76),7例患者p53,p27,bcl-2均过度表达。p53阳性表达率随分化程度降低而升高,bcl-2和p27阳性表达率随分化程度降低而降低,且高、低分化组间差异均有显著性意义(P<0.05)。但是三种蛋白阳性表达率均与肺癌组织学类型、淋巴结转移及病理分期无明显关系(P>0.05)。结论:p53,p27和bcl-2基因过度表达可能与非小细胞肺癌的发生、发展有关。 BACKGROUND:The quality of life(QOL) of patients with lung cancer is affected d ue to depression,reduced lung function,subjective reduced body force,fatigue,and poor stamina,etc.,and the survival of the patients would be affected by complic ations or advanced stage cachexia. OBJECTIVE:To investigate the relationship between three gene proteins includin g p53,p27 and bcl 2 and the pathological characters of non small cell lung can cer(NSCLC). DESIGN:An experimental trial by employing pathological specimens as subjects. SETTING:Department of respiration of a university affiliated hospital and the Central laboratory of a university. PARTICIPANTS:Totally 76 specimens of NSCLC after surgical resection between Ju ne 1997 and December 2002,which were all primary lung cancer without any other t herapy. METHODS:The expression of three gene proteins in 76 NSCLC specimens was detect ed by SP immunohistochemical analysis. MAIN OUTCOME MEASURES:Positive expression of p53,p27 and bcl 2. RESULTS:Among 76 specimens,28 cases(37%, 28/76) with excessive expression of p53,34 cases(45%, 34/76) with excessive expression of p27,37 cases(49%, 37/76) with excessive expression of bcl 2,and 7 cases with excessive expression in al l three proteins. The positive expression of p53 elevated with the reduced grada tion in differentiation;bcl 2 and p27 positive expressions reduced with the red uced gradation in differentiation and there was significant difference between h igh differentiation group and low differentiation group(P< 0.05).However,there was no significant relationship between the positive expressions of three prote ins and the histological classification,lymph node metastasis, and pathological aging of lung cancer(P >0.05). CONCLUSION:The excessive expression of p53,p27 and bcl 2 genes might be relat ed with the occurrence and development of NSCLC.
