十二指肠憩室的多层螺旋CT诊断价值

Evaluation of multislice helical computed tomography in the diagnosis of duodenal diverticulum.

目的:探讨多层螺旋CT对十二指肠憩室的诊断价值。方法:对6 5例经上消化道钡餐检查确诊十二指肠憩室患者进行多层螺旋CT扫描,分析其CT表现,并利用仿真内镜技术显示憩室。结果:80个十二指肠憩室多层螺旋CT检查出15个,均大于1.0cm ,小于1.0cm的憩室未检出。憩室表现为胰头右后方半圆形含气体腔影,可有液平,与胰头交界部边缘锐利。仿真内镜技术(CTVE)可显示憩室口及憩室内壁。结论:多层螺旋CT对十二指肠憩室的检查敏感性较低,尽管三维仿真内镜对诊断憩室有一定作用。

Objective:To evaluate the value of multislice helical computed tomography (MSCT) scanning in the diagnosis of duodenal diverticulum.Methods:MSCT scanning were performed on 65 patients with duodenal diverticulum. Then CT manifestations were analyzed.Results:Of 80 cases duodenal diverticulum ,only 15 cases were clearly detected which size was beyond 1cm,none of duodenal diverticulum which size smaller than 1cm was detected.Duodenal diverticulum manifested as hemicycle lesion with gaseous density which had air-fluid level behind pancreatic head with clear border.The neck and interior wall of duodenal diverticulum were manifested by CT virtual endoscopy(CTVE).Conclusion:MSCT has very low sensitivity in the detection of duodenal diverticulum,Despite CTVE is clinically useful for the evaluation of duodenal diverticulum,CT is not a choice to be a method of general examination.
