七叶皂苷治疗四肢创伤及其手术后肿胀的疗效和安全性

Efficacy and safety of aescine in treating post-traumatic and operative swellings of limbs

目的:观察七叶皂苷治疗四肢创伤及其手术后肿胀的疗效和安全性。方法:采用随机对照试验。试验组男性46例,女性26例,年龄(44±s13)a,予七叶皂苷15～20mg·d-1;对照组男性16例,女性8例,年龄(43±11)a,予丹参800mg·d-1。均溶于5%葡萄糖注射液500mL中,iv,gtt,qd,疗程7d。结果:给药后,2组肿胀积分均显著减少,试验组下降(2.1±0.7)分,对照组下降(1.5±0.8)分,均P<0.01,2组差异显著(P<0.05)。试验组总疗效和对重度肿胀的疗效优于对照组(P<0.05,P<0.01)。试验组有3例ALT升高、6例BUN和(或)Cr升高、1例蛋白尿、3例血钾下降、3例舒张压下降。结论:七叶皂苷治疗四肢创伤及其手术后肿胀有效,对有肝、肾功能损害者,有低血钾者,有低血压者应在监测下应用。

AIM: To evaluate the efficacy and safety of aescine on swelling after limbs' wound and operation. METHODS: A randomized controlled trial was adopted. The treatment group(M 46,F 26; age((44±)s 13) a) was administrated with 15-20 mg·d^(-1) aescine and the control group(M 16,F 8; age(43±11) a) was treated with 800 mg·d^(-1) Salvia miltiorrhiza. Both agents were dissolved in 5 % glucose (500 mL) for injection, iv, gtt, qd, for 7 d. RESULTS: The accumulating scores of swelling of both groups were significantly reduced after the treatment (P<(0.01)). The decreasing in values of treatment group was (2.1±0.7) and that of control group was ((1.5±0.8)),P<0.05. The total effective rate and efficacy for the severe swelling of treatment group was superior to those of control group (P<0.05, P<0.01). In treatment group, 3 patients showed ALT rising, 6 patients with BUN(Cr) rising, 1 patient with proteinuria, 3 patients of hypopotassemia, 4 patients with significant diastolic pressure lowing. CONCLUSION: Aescine has excellent efficacy on diminishing swelling after limbs' wound and operation, but attention should be taken for patient with impaired renal and hepatic function, hypopotassemia and lower blood pressure.
