摘要
目的:卡氏肺孢子虫肺炎(PCP)是获得性免疫缺陷综合征(AIDS)等免疫损害宿主最常见的呼吸系统感染性疾病,其发病机制仍未完全清楚。研究大鼠卡氏肺孢子虫感染后血清中酶学指标的变化,并观察大鼠肺脏、肝脏的病理变化及肺泡内的PC ,探索PCP的发病机制。方法:用清洁级雌性wister大鼠5 0只,体重2 2 0~2 6 0 g。随机分正常对照组(N组)、卡氏肺孢子虫肺炎组(PCP组)、复方新诺明治疗组(SMZ组)。PCP组、SMZ组饮含0 .5 g/L盐酸四环素的冷开水抗细菌感染,N组饮冷开水。PCP组、SMZ组每周2次皮下注射地塞米松,每次2 5mg/kg体重,N组皮下注射相同体积的注射用水。从第3周开始,对SMZ组进行复方新诺明灌胃治疗,用量为2 5 0mg/kg .d ,5d为一疗程,以后每隔2周灌胃一个疗程。第12周剖杀,心脏采血,测定血清中γ-谷氨酰转移酶(γGT)、总胆汁酸(TBA)、胆碱脂酶(ChE)、肌酸激酶(CK)、肌酸激酶同工酶(CKMB)、羟丁酸脱氢酶(HBDH)、淀粉酶(AMY)、α-L -岩藻糖苷酶(AFU)、5′核苷酸酶(5′NT)、腺苷脱氨酶(ADA)。结果:PCP组TBA比N组显著降低(P <0 .0 5 ) ,CK、CKMB、HBDH、AMY、ADA比N组极显著降低(P <0 .0 1) ,而AFU比N组显著升高(P <0 .0 1) ;SMZ组TBA、CHE、CKMB、ADA比N组显著降低(P <0 .0 5 ) ,CK、AMY比N组极显著降低(P <0 .
Objective: Pneumocystis carinii pneumonia(PCP) is a common infectious disease of respiratory system for immune impaired host, including aquired immune deficiency syndrome(AIDS),and its pathogensis is yet to beunderstood. This study was to explore the changes of enzymes in serum of the rats infected with Pneumocystis carinii, and observe the pathological changes for the pulmonary and liver and the PC in alveoli of pulmonary of rats, so as to explore pathogensis of PCP. Methods:50 Wistar rats were randomly divided into normal group(N group), PCP model group(PCP group) and TMP-SMZ therapy group(SMZ group),body mass being 220~260g. Rats in PCP group and SMZ group were given cold boiled water, containinging 0.5 g/L tetracycline in order to resist bacterial infection;Rats in N group were given boiled water. Rats in PCP group and SMZ group were 25 mg/kg gravity dexamethas with hypodermic injection, twice every week. N group was done in the same way. From the third week, SMZ group was treated by infusing the stomach with the medicine, 250mg/kg.d, sequential 5 days forming a course of treatment; and then the same process was done every two weeks. All the rats were killed in twelfth week, blood collected from heart and γGT, TBA, ChE, CK, CKMB, HBDH, AMY, AFU, 5′NT and ADA examind.Results: The TBA level of PCP group was notably lower than N group(P<0.05). The CK, CKMB, HBDH, AMY and ADA levels of PCP group were notably lower than N group(P<0.01),but the AFU level of PCP group was extremely higher than N group(P<0.01). The TBA,CHE,CKMB,ADA level of SMZ group was notably lower than N group (P<0.05),The CK and AMY level of SMZ group was notably lower than N group (P<0.01),but the AFU level of SMZ group was higher notably than N group(P<0.01). There was no significant difference in CK, CKMB, HBDH, ADA between PCP group and SMZ group (P<0.01).Conclusion:The enzyme changes of rats' serium reflected that the inflammary changes of pulmonary tissue for rats made the enzymes such as TBA, ChE, CK, CKMB,HBDH, AMY, AFU and ADA significantly variable after they were infected with Pneumocystis carinii, but there was no significant difference in γGT and 5'NT levels between experimentary group and normal group(P>0.05).
