摘要
目的:探讨Tc-99-MDP(以下简称“云克”)治疗对骨质疏松动物模型的细胞学及骨密度的变化和临床银屑病性所致的骨性关节炎的初步疗效分析。方法:取新西兰实验白兔32只,分为正常对照组、骨质疏松对照组和骨质疏松“云克”治疗组,每组8只。除正常对照组外,其他组以3mg/kg剂量DX肌肉注射,每周二次,连续6周后对进行腰椎X线摄片、CT摄片、放射性核素ECT显像检查,了解骨质疏松情况。以后对“云克”治疗组进行“云克”治疗16周,再进行X线摄片、CT摄片、放射性核素骨骼显像,处死各组实验兔后,去除软组织,取实验兔左股骨头、第3腰椎进行骨细胞学检查,右股骨头部位、第4腰椎骨进行骨密度测定和骨形态计量分析。对临床明确诊断为银屑病皮疹与关节病变共存的银屑病性关节炎(PA)患者男性8例,女性7例,平均年龄41.7岁。15例患者均表现为远端指关节病变,其中9例以腕、掌指关节为主,6例以指关节为主。曾用雷公藤多甙片治疗,另附加芬必得片辅助镇痛治疗,但症状不能缓解,然后进行“云克”治疗。在治疗前、后每例患者行血沉、CRP测定,病变关节X线摄片、放射性核素99Tcm-MDP骨显像及临床症状评判疗效。结果:骨质疏松模型组肌肉注射DX第6周,放射性核素骨骼显像显示正常组为骨骼显影清晰,放射性分布对称、均匀。骨质疏松模型组骨?
Objective: Investigating the changes in cytology and bone density of the animal model in which the animals suffered from osteoporosis have been treated with 'yuenke' (YuenKe) and initial analyzing the curative effects of arthritic of psoriasis type treated with 'yuenke'.Method: 32 New Zealand experimental white rabbits were separated into normal compare group, compare group of osteoporosis and “YuenKe” curing group of osteoporosis. Every group had eight rabbits. Except the compare group, dexamethasone (DX) was injected into other group rabbits muscle 3mg per kg twice a week and six weeks altogether. In order to understand situation of osteoporosis, the rabbits injected with DX were shot with X rays, CT and the video pictures of nuclein ECT were checked. Then the rabbits of “YuenKe” curing group were under treatment for 16 weeks. We took the photographs of X rays, CT and neclein video pictures again. After killing rabbits of all groups and clearing away their soft tissue, we made the inspects of bone cytology of left thigh-bones and third lumbar vertebras and the calculable analyses of bone formation of right thigh-bones and fourth lumbar vertebras as well as measured their bone density.15 patients, eight among them being man, were diagnosed exactly as PA patients who contracted psoriasis rash and joint pathology at a time. Their average age was 41.7 years old. They all suffered from far-end finger joints pathology. Nine of them mainly suffered from carpus and palmar joints and the left mainly suffered from finger joints. They were cured with LEIGONGTENG and FENBIDEwhich can ease pain for assist treatment, but failed to get respond to medical treatments. Then they were treated with“YuenKe”. Before and after this treatments, we tested blood sedimentation and CRP and shot photographs with X ray, made the skeleton video pictures by isotope 99Tc m-MDP and judged the curative effects according to clinical symptom for each patient.Result: The rabbits of osteoporosis group underwent muscle injection till sixth week, the skeleton video pictures of nuclein show that the video pictures of normal group are clear and radioactivity scatters symmetrically and evenly. The skeleton video pictures of osteoporosis model group are vague and indistinct, but the radioactivity on the sections of four limbs’ big joints strengthens and concentrates. The pictures of X rays and CT don’t change clearly. After sixteenth weeks, comparing with normal group, the thigh-bones osteoporosis group are destroyed, the bone trabeculas become thin and bone blast cells decrease clearly. The examining results of thigh-bone cytology of “YuenKe” group is similar to that of normal group. We analyze in ration the bone density, bone form and examine the ROI ratio of nuclein video pictures of fourth lumbar vertebra and left thigh-bone of tested rabbits of all groups. We find they exist obviously differences.After 16 weeks treatment there are different improvements in patients’clinical expression, pain symptoms, blood sedimentation test in laboratory, CRP test and wrist joints’ bone video picture ROI with isotope 99Tc m-MDP.Conclusion: “YuenKe” may be possesses the functions of adjusting body’s immune function, restraining the activity of clasmaloeyte, decreasing the loss of bone calcium and promoting sclerotin proliferation. The animal tests, the ration curative results of fifteen patients suffered from arthritic of psoriasis type cured with “YuenKe”, the pathology cytology analyses of osteoporosis animal model, the calculating number of density and bone form and the isotope scanning of skeleton show that “YuenKe” can revamp the bone tissue distinctly and improve the patient symptom of the psoriasis arthritis.
