介入化疗栓塞联合放疗治疗中晚期肝癌的临床观察

Combined Therapy of Interventional Chemotherapy,Embolism and Radiotherapy in the Treatment of Advanced Iiver Cancer

目的:观察介入化疗及栓塞术联合放射治疗中晚期肝癌的近期疗效及副作用。方法:6 0例不能手术切除的晚期肝癌患者(TNM分期Ⅳ期)随机分为两组:①介入化疗栓塞法。②介入化疗栓塞法联合放疗法。全部患者随访3～2 4个月,平均8 .2 3±2 . 76个月。分别比较两组患者的中位生存时间,6个月、1年生存率、死亡患者平均生存时间。结果:介入法联合放射治疗组中位生存时间为11 6个月,6个月和1年生存率分别为82. 4 %和4 3. 8% ;而介入法组患者中位生存时间为4 .2个月,6个月和1年生存率分别为15 . 2 %和0 ,两组比较差异有非常显著性(P <0 . 0 1)。介入法联合放射治疗组死亡患者的平均时间为10 . 2 1±4 . 16个月,而介入组患者为4 . 12±2 . 17个月,两组比较,差异有非常显著性(P <0. 0 1)。结论:介入法联合放射治疗晚期肝癌患者的疗效明显优于单纯介入法组,为治疗中晚期肝癌的一种有效治疗手段。

Objective:To observe short-term t he rapeutic effect and side-effect on the combined therapy of interventional chemot herapy,embolism and radiotherapy for advanced liver cancer.Methods: 60 cases of advanced liver cancer were randomly divided into two groups.Inter vention group was treated with interventional chemotherapy and embolism.Combined treatment group was treated with interventional chemotherapy,embolism and radio therapy.Duration of follow-up was 8.23±2.76 months (range,3 months to 24 month s).the median surviral time,6-month or 1-year survival rate and average survival time between two groups were compared.Results:In combined treat ment group and intervention group,the median survival time was 11.6 months and 4 .2 months,respectively;6-month and 1-year survival rate was 82.4% and 43.8%,15.2 % and 0,respectively;the average survival time was 10.21±4.16 and 4.12±2.17 mo nths,respectively.The difference was markedly significant (P<0.01).Conclusion:The efficacy of interventional chemotherapy combined with radiotherapy is notably superior to that of intervention.It is an effective therapy for advanced liver cancer.
