摘要
目的 探讨vWF裂解蛋白酶(ADAMTS 13,vWF cp)活性水平的检测在系统性红斑狼疮(SLE)中的临床意义。方法 采用残余胶原结合实验(residual collagenbindingassay)对38例SLE患者血浆ADAMTS 13活性水平进行检测,同时以6例血栓性血小板减少性紫癜(TTP)患者为阳性对照,以32例健康人为正常对照。结果 SLE及TTP患者血浆ADAMTS 13活性水平[(71.34±12 .85 ) %、(12 .5 4±11.6 9) % ]显著低于正常人[(78.6 0±10 .0 3) % ](P <0 .0 1) ,SLE血浆ADAMTS 13活性水平明显高于TTP患者(P <0 .0 1) ,SLE血浆ADAMTS 13活性水平阳性率[18.4 2 % (7/ 38) ]明显低于TTP患者[10 0 % (6 / 6 ) ](P <0 .0 1)。结论 ADAMTS 13活性水平检测对研究SLE血栓形成过程具有重要意义,也可能有助于SLE与TTP的鉴别诊断。
Objective To explore the clinical significance of the detection of von Willebrand factor cleaving protease (ADAMTS-13, vWF-cp) activity in systemic lupus erythematosus (SLE) Methods ADAMTS-13 activity were evaluated by using Residual-Collagen Binding Assay in 38 patients with SLE, 6 patients with thrombotic thrombocytopenic purpura(TTP) and 32 normal controls. Results The level of ADAMTS-13 activity in the SLE and TTP patients[(71.34±12.85)% (12.54± 11.69)%] were significantly lower than that in the control subjects[(78.60±10.03)%] (P< 0.01), the level of ADAMTS-13 activity in the SLE was significantly higher than that of TTP(P< 0.01), the positivity rate of the ADAMTS-13 activity in the patients with SLE[18.42%(7/38)] was significantly lower than that of TTP (P<0.01). Conclusion Detecting ADAMTS-13 activity is meaningful to study thrombosis procedure in the SLE patients, meanwhile, it might be helpful for differentiating SLE and TTP.
