老年结直肠癌与非老年结直肠癌704例临床分析

Characteristics of colorectal cancer in the elderly: A retrospective study of 704 surgical cases

目的 对2000年至2002年我院腹部外科收治的年龄≥60岁结直肠癌315例与同期住院的非老年结直肠癌患者389例进行回顾性对比分析,了解老年大肠癌临床特点。方法 所有结直肠癌均经手术治疗,病理证实,采用根治性、姑息性或减状手术治疗方法。结果 老年组腹部肿块、肠梗阻及全身症状、并存疾病发生率较非老年组高,癌肿分化较好,分期较早。结论 老年结直肠癌发病率高,症状体征、组织类型、发病部位、分期与非老年结直肠癌有所不同。

Objective To determine the characteristics of colorectal cancer in the old patients through a retrospective study of 704 patients with colorectal cancer.Methods All patients experienced operation and had pathologic findings.Results There were significantly differences in abdominal mass, bowel obstruction, systemic syndromes and complications between two groups. In the old group, abdominal mass, bowel obstruction, systemic syndromes and complications had higher ratio compared with those in the young groups,P<0.001. Higher proportions of better-differentiated and earlier stage cancer were present in the old group, P<0.001.Conclusion Elderly patients with colorectal cancer have several distinct clinicopathological features. They showed higher incidence and marked difference in cancer location, clinical syndromes and histological type.
