剖宫产孕妇留置双腔导尿管气囊内注液量的探讨

目的减轻孕妇留置导尿管后的不适感,减少剖宫产术后肉眼血尿的发生率。方法对2004年9月择期剖宫产孕妇87例(对照组),术前留置导尿时气囊内注射生理盐水10ml;2004年10月择期剖宫产孕妇98例(观察组),留置导尿时气囊内注射生理盐水5ml。观察两组产妇留置导尿管后的不适感包括尿道痛、反复尿意、下腹坠胀痛,剖宫产术后肉眼血尿及导尿管滑脱情况;记录结果采用字2检验进行统计学处理。结果观察组孕产妇比对照组孕产妇留置导尿管后的不适感减轻,术后肉眼血尿的发生率明显下降(P<0.01),有显著性差异且不增加导尿管滑脱率。结论适当减少双腔气囊导尿管气囊的注水量,可以减轻置管后的不适感,减少术后肉眼血尿的发生率。

Objective To alleviate the discomfortness of pregnant women with indwelling catheter and to reduce the occurrence rate of gross hematuria. Methods In control group consisting of 87 pregnant women before cesarean who were selected in Sep. 2004, balloons were injected 10 ml saline when indwelling catheter pre-operatively. In observation group consisting of 98 pregnant women before cesarean who were selected in Oct. 2004, balloons were injected 5 ml saline. Discomfortness in two groups, including pain of urethra, frequent urge for urination, abdominal distention and pain, gross hematuria, and dislocation of catheter were observed, with all recorded results being tested by ?字2. Results Pregnant women in observation group had less discomfortness after indwelling catheter than those in control group, and the occurrence rate of gross hematuria was significantly lower (P<0.01), without increasing the dislocation rate of catheter. Conclusion To reduce the volume of saline injected into the double lumen balloon can alleviate the discomfortness after indwelling catheter and reduce occurrence rate of post-operative gross hematuria.
