曲线M型组织速度图与应变率成像定量评价局部心肌功能的初步研究

摘要 目的 初步探讨组织多普勒曲线M型速度图(CMM TVI)和应变(Strain)、应变率(strainrate ,SR)显像新技术对定量检测局部心肌功能的可靠性。方法 通过超声多普勒组织成像的TVI和SRI成像模式,结合同步记录的心电图,分别观察及测量2 0例健康成年人和14例房间隔缺损(ASD)患者心尖四腔观室间隔和右室游离壁各节段心肌在不同时相的运动速度及应变、应变率。结果 ①正常组室间隔和右室游离壁的运动速度、应变及应变率从基底到心尖呈下降趋势;②与正常对照组比较,ASD患者右室游离壁各节段的运动速度及应变、应变率均大于正常组,P <0 .0 5。结论 组织多普勒曲线M型的速度和应变率显像是一种可用于评价局部心肌功能的超声影像新技术,可以为患者的临床治疗和估计预后提供重要的信息。 Objective To dicuss the reliability of regional myocardial functional quantitative measurement using the new technologies of Doppler tissue curved M-Mode velocity imaging (CMM-TVI) and strain,strain rate (SR) respectively.Methods Twenty healthy subjects and fourteen ASD patients were studied at apical four-chamber view using TVI and SRI under synchronized electrocardiography.Each of apical,middle and basal segment of intraventricular septal (IVS) and right ventricular free wall (RVFW) was observed and quantitatively assessed in every phases.Results ①In the normal group,velocities,strains and strain rates of IVS and RVFW showed descend trend from basal segments to apical segments.②Compared with normal group,ASD patients had higher values of velocities,strain and strain rate of RVFW,P<0.05.Conclusions Doppler tissue curved M-Mode velocity and strain rate imaging is a new echocardiographic technology to evaluate regional myocardiac function and will play an important role in clinical treatment and estimate of prognosis.
