动力轴位旋转X线检查在颈性眩晕中的应用价值 被引量:15

摘要 目的:探讨动力轴位旋转X线检查在颈性眩晕诊断和治疗中的应用价值。方法:对180例颈性眩晕的患者进行动力过屈过伸位和动力轴位旋转X线检查并对下颈椎不稳患者采用颈围外固定、封闭等综合保守治疗,症状反复发作者行颈前路融合固定。结果:X线检查表明,下颈椎不稳141例,占78%,其中矢状位不稳119例,占66%,轴位旋转不稳95例,占52%,73例患者矢状位和旋转不稳同时并存。保守治疗优良率为92%,手术治疗优良率为100%。结论:动力轴位旋转X线检查可以发现下颈椎轴位旋转不稳,对颈性眩晕的诊断和治疗具有重要价值。 Objective: To study the significance of cervical dynamic axial rotatory radiography in diagnosis and treatment in cervical vertigo. Methods: One hundred and eighty patients with cervical vertigo were analyzed on routine, dynamic sagittal and dynamic axial radiography. The patients with lower cervical instability received conservative treatments fristly. If conservative treatments were inefficient, the patients were performed anterior fusion and plating. Results:One hundred and forty-one cases were found with lower cervical instability(78%), 119 cases with sagittal instability(66%), 95 cases with axial rotatory instability(52%). 73 cases with both sagittal and axial instability. The conservative curative effect in lower cervical instability patients were graded as excellent in 94 cases (67%)and good in 34 cases (24%); the anterior fusion and plating effect was graded as excellent in 9 cases (75%) and good in 3 cases (25%). Conclusion: The dynamic axial rotatory radiography can determine axial instability in lower cervical, and do great significance for cervical vertigo.
