妊娠高血压综合征患者血清瘦素和血脂的变化

Changes About the Levels of Leptin and Blood Lipids in Pregnancy-induced Hypertension Syndrome

目的 探讨妊娠高血压综合征(妊高征)患者血清瘦素、血脂水平变化及其关系。 方法 用ELISA、蛋白酶法测定5 6例妊高征患者产前、产后血清瘦素、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL C)、低密度脂蛋白(LDL C)、载脂蛋白A(ApoA)、载脂蛋白B(ApoB) ;2 5例相同孕周的正常孕妇为对照组。 结果 (1)妊高征组体重指数(BMI)、血压和血清尿素氮、尿酸、肌酐、红细胞压积显著高于对照组(P <0 .0 5 ) ,新生儿出生体质量显著低于对照组(P <0 .0 5 ) ;(2 )妊高征组TC、TG、LDL C/HDL C、ApoB、瘦素高于对照组,HDL C、ApoA/ApoB低于对照组,差异有显著性(P <0 .0 5或P <0 .0 1) ;轻、中、重度妊高征血清TC、LDL C、ApoB、瘦素水平逐渐升高,差异有显著性(P <0 .0 5或P <0 .0 1) ;(3)血清瘦素与BMI无显著相关性,与收缩压、舒张压、平均动脉压呈正相关;血清瘦素与TC、TG、ApoB呈正相关;(4)妊高征患者产后血清瘦素水平显著降低,差异有显著性(P <0 .0 1)。 结论 妊高征患者存在一定程度脂代谢变化;瘦素可能通过影响血脂代谢而参与妊高征发生。

Objective To analysis the variation and relationship of the leptin and blood lipid in pregnancy-induced hypertension syndrome(PIH) patients. Methods The plasma leptin, TC, TG, HDL-C, LDL-C, ApoA, ApoB were detect by ELISA and proteinase methods,in 56 patients with pregnancy-induced hypertension syndrome. 25 cases normal pregnant women with a same pregnant weeks,as control. Leptin, TC, TG, HDL-C, LDL-C, ApoA, ApoB were detected with ELISA and proteinase examination methods. Results (1)The levels of body mass index, blood pressure, BUN, UA, Cr, HCT in PIH group are significantly higher than the control group(P<0.05). and the newborns' weight in PIH group are lower than the control group significantly(P<0.05). (2)The levels of TC, TG, LDL-C/HDL-C, Apo-B, Leptin in PIH group are significantly higher than the control group, and the levels of HDL-C, Apo-A/Apo-B are lower(P<0.05 or P< 0.01 ). The levels of TC, LDL-C, Apo-A significantly increase with the degree of hypertension(P<0.05 or P<0.01). (3)The level of blood leptin don't have a significant correlation with BMI, LDL-C, HDL-C, Apo-A, but have a positive correlation with systolic pressure, diastolic pressure , mean arterial pressure and the levels of TC, TG,Apo-B. (4)The PIH leptin level decrease significantly after birth(P<0.01). Conclusion There are often different degree of lipid metabolism disorder in PIH patients. Leptin may cause the occurrence of PIH by affecting of the lipid metabolism.
