微波组织凝固治疗口腔癌的机制及临床意义

摘要 目的 探讨微波组织凝固(MicrowaveTissueCoagulation ,MTC)治疗口腔癌后引起细胞凋亡、免疫增强的机制。方法 采用MTC方法治疗原发灶,治疗前后辅以平阳霉素化疗,颈部行舌骨上清扫或全颈淋巴结清扫,治疗后取标本作光镜、透射电镜观察细胞形态学变化。随访6~2 4个月。结果 经MTC治疗后病理报告为变性坏死组织、炎性肉芽、渗出坏死、增生或不典型增生。电镜下可见大量细胞凋亡现象及肥大细胞。结论 MTC可杀灭口腔癌原发灶,同时诱导邻近组织细胞凋亡和免疫增强来控制肿瘤的局部复发。 Objective To study the morphologic changes of cancerous tissues and the correlations among microwave tumor coagulation (MTC), apoptosis, and immunoenhancement.Methods The primary focus of tumor was given MTC therapy, combined with chemotherapy and/or lymphadenectomy of the suprahyoid region. After MTC, the cancer tissues were extracted immediately for pathological and morphological examination using a transmission electron microscope (TEM). All patients were followed for 6 months to 2 years and the therapeutic effects were evaluated.Results After MTC treatment, the tissue showed degeneration and necrosis, inflammatory granulation, exudative necrosis, and hyperplasia or atypical hyperplasia.Apoptosis and mastocytes were observed under the TEM.Conclusion Microwave tissue coagulation can obliterate the primary focus of oral carcinoma and can control the local recurrence of tumor through evoking apoptosis and enhancing immunity.
