核素骨显像诊断颞下颌关节病变的临床价值 被引量:11

摘要 目的:探讨利用放射性核素骨三相显像和SPECT检查颞下颌关节紊乱病患者的价值。材料和方法:6 6例颞下颌关节紊乱病患者接受X线片、放射性核素骨三相显像和SPECT检查。通过骨扫描结果进行定性和半定量分析,选择2 3例患者接受外科手术治疗。结果:选择保守治疗者4 3例,血池相和延迟相影像及SPECT两侧关节区影像对称。接受颞下颌关节重建术者2 3例,血池相和延迟相影像及SPECT影像两侧关节区影像不对称,患侧关节区放射性强度和髁状突放射性计数比值稍高于对侧;术中发现,髁状突表面软骨有破坏,术后患者症状缓解。结论:骨扫描可以作为颞下颌关节紊乱病的早期器质性病变的检测手段,选择为治疗方法提供参考。 Purpose:To evaluate three-phase bone scan and single photon emission computed tomographry(SPECT) in management of temporomandibular joint disorder. Materials and Methods: Three-phase bone scan,SPECT and X-ray images over temporomandibular joints of 66 patients with temporomandibular joint disorder were undertaken. 23 patients underwent surgery based on the abnormal findings in semi-quantitative and qualitative analysis of the scans. Results :The X-ray showed no bony injury in all cases in spite of their symptom.Asymmetrical blood flow, bony uptake in 23 patients was in accordance with condylar cartilage destroy found during the surgery. Conclusion: Bone scintigraphy and SPECT are sensitive and accurate in detecting early stage of bone damage in temporomandibular joint disorder, offering references for treatment planning for those patients.
