探讨B超诊断阑尾炎分型的价值

Ultrasonographic Value in the Diagnosis of Appendicitis’ Classification

目的:探讨阑尾炎的B超图像特征及其分型。方法:75例经手术及病理证实的阑尾炎,对其B超图像特征进行分析并加以分型。结果:急性阑尾炎B超图像特征均显示阑尾增粗,成人直径大于0.7cm,儿童直径大于0.6cm,粘膜增粗、中断或缺损,回声明显减弱。根据图像特征将其分为:①单纯性阑尾炎(包括慢性阑尾炎2例);②化脓性阑尾炎;③坏疽性阑尾炎;④阑尾周围脓肿;⑤急性阑尾炎合并间接征象。结论:B超对阑尾炎的诊断及分型具有重要的临床意义。

Objective: To explore the ultrasonographic characteristics of appendicitis and its classification. Methods: Seventy-five patients underwent ultrasonographic examination and diagnosed as appendicitis, then the characteristics of ultrasonography were analyzed, classified and correlated with the histologic diagnosis, respectively. Results: All the ultrasonographic findings showed that the appendixes were thickened with the diameter >7mm in adults and with the diameter >6mm in children, and the mucous membrane was coarse with echo interrupted or absent. According to the ultrasonographic features of appendicitis ,five styles were divided: ①simple appendicitis including 2 chronic appendicitis;②purulent appendicitis;③gangrenous appendicitis;④appendiceal abscess;⑤appendicitis with indirect characteristics.Conclusion: Ultrasonography is one of high clinical significant for diagnosis and classification of appendicitis.
