^1H磁共振波谱在脑多发性硬化诊断中的价值 被引量:3

Value of 1H Magnetic Resonance Spectroscopy in Diagnosis of Multiple Sclerosis in Brain

摘要 目的 结合MRI探讨1H磁共振波谱(1HMRS)在脑多发性硬化(MS)诊断中的临床应用价值。方法 选择32例经临床确诊为MS患者进行MR及1HMRS检查,同时与同期同年龄组MRI显示正常者进行对照研究,通过计算峰下面积,对代谢产物氮乙酰天门冬氨酸(NAA)、肌酸(Cr)、胆碱复合物(Cho)及乳酸(LAC)的浓度进行定量并观察NAA/(Cr+Cho)、Cho/Cr及LAC/Cr等各参数的变化。结果 病变组病例NAA/(Cr+Cho)、Cho/Cr及LAC/Cr值分别为0. 53、1. 47及0. 23,对照组分别为0. 75、1. 19及3. 35。MS病人NAA/(Cr+Cho)值下降,差异有显著性(t值为4. 76,P<0.05),Cho/Cr、LAC/Cr值升高,差异有显著性(t值分别为2. 36、4. 11,P均<0. 05)。结论 1HMRS和MRI互相补充有利于提高MS的诊断。 Objective To evaluate the clinical value of ~1H magnetic resonance spectroscopy(~1H-MRS) in the diagnosis of multiple sclevrosis(MS) in brain.Methods ~1H-MRS was carried out in 32 patients with MS and 32 age-matched healthy control subjects.By calculating the area below peak,the metabolite N-acetylasparatate(NAA),creatine(Cr) choline compouds(Cho),Cho/Cr and lactate(Lac)/Cr were quantified and their changes were observed.Results NAA/(Cr+Cho),Cho/Cr and Lac/Cr were 0.53,1.47 and 0.23 in MS group,and 0.75,1.19 and 3.35 in control group.NAA/(Cr+Cho) decreased in MS group.The difference between 2 groups was statistically singnificant(t=4.76,P<0.05),Cho/Cr and LAC/Cr increased significantly in MS group(t=2.36,4.11,P<0.05). Conclusions The complementation of ~1H-MRS and MRI is helpful to improve diangnostic accuracy of MS.
