反弧形小切口非超声乳化人工晶状体植入手术体会

Cataract surgery of small scleral in-cision with ends curving away from corneal limbus

目的:评价反弧形小切口非超声乳化人工晶状体植入手术的疗效。方法:对400例(486眼)白内障施行反弧形小切口非超声乳化人工晶状体植入手术。结果:术后1d矫正视力≤0.3者18眼占3.7%,0.3～0.5者359眼占73.9%,0.5以上109眼占22.4%。术后1wk矫正视力≤0.3者6眼占1.2%,0.3～0.5者289眼占59.5%,0.5以上者191眼占39.3%。术后3mo矫正视力≤0.3者3眼占0.6%,0.3～0.5者125眼占25.7%,0.5以上358眼占73.7%。术中并发症主要为后囊膜破裂,瞳孔损伤;早期并发症主要为角膜水肿,虹膜炎;晚期并发症主要为后发性白内障。结论:反弧形小切口非超声乳化人工晶状体植入手术安全易行,不需要添置昂贵的医疗设备,值得在基层医院推广应用。

· AIM: To evaluate the effect of cataract surgery of small scleral incision with ends curving away from corneal limbus.· METHOD: A total of 400 cataract patients (486 eyes) were performed with cataract removal and intraocular lens implantation in small scleral incision with ends curving away from corneal limbus.· RESULTS: At postoperative 1d, vision acuity was corrected to ≤0.3 in 18 eyes(3.7%), between 0.3 and 0.5 in 359 eyes (73.9%) and better than 0.5 in 109 eyes(22.4%). At 1 wk,after the surgery it was ≤0.3 in 6 eyes (1.2%), between 0.3 and 0.5 in 289 eyes (59.5%) and better than 0.5 in 191 eyes (39.3%). At post operative 3mo, it was ≤0.3 in 3 eyes (0.6%), between 0.3 and 0.5 in 125 eyes (25.7%) and better than 0.5 in 358 eyes (73.7%). The major intraoperative complications included rapture of posterior capsulae and pupil injuries; the early postoperative complications included corneal edema and iritis; The late postoperative complication was mostly after cataract.· CONCLUSION: Cataract surgery of small scleral incision with ends curving away from corneal limbus is safe and easy without needs for expensive equipments. Therefore it is worth being widely used in hospitals of rural areas.·
