米氮平治疗老年不稳定型心绞痛伴抑郁症临床初评

Curative Effect of Mirtazapine for Senile Unstable Angina Pectoris Combined with Depression

目的:探讨抗抑郁药米氮平辅助治疗老年不稳定型心绞痛伴抑郁症患者的临床疗效。方法:6 8例老年不稳定型心绞痛伴抑郁症患者,随机分为治疗组、对照组各34例。对照组予常规心血管药物治疗,治疗组在常规心血管药物治疗的基础上加米氮平30mg/d。观察期均为6个月。结果:治疗组2 9例心绞痛发作每周少于或等于1次、5例为每周2次以上,对照组则分别为9例、2 5例,两组比较差异均有统计学意义(P <0 . 0 1 )。治疗组每周硝酸甘油用量少于或等于2mg、2mg以上的患者分别为2 7例、7例,对照组则分别为1 0例、2 4例,两组比较差异均有统计学意义(P <0 . 0 1 )。治疗组治疗3、6个月时心肌缺血(ST段压低次数)分别为( 73±1 5)次、( 51±1 5)次,对照组则分别为( 1 0 1±2 3)次、( 1 1 8±2 4 )次,两组比较差异均有统计学意义(P <0 0 5)。治疗组再次住院2例,对照组7例,两组比较差异有统计学意义(P <0 . 0 5)。结论:对老年不稳定型心绞痛伴抑郁症患者,加用米氮平抗抑郁治疗效果良好。

Objective: To evaluate the clincal effect of mirtazapine for supportive treatment of senile unstable angina pectoris combined with depression.Methods:68 cases of senile unstable angina pectoris combined with depression were divided into two groups.The control group was treated with routine drugs,and the treated group was given mirtazapine(30mg/d),in addition to the routine treatment.The duration of observation was 6 months in two groups.Results:In treatment group there were 29 cases breaking out one or less time and 5 cases breaking out more than 2 times weekly. In control group there were 9 and 25 cases having aforementioned conditions respectinely.The difference in two groups was significant( P <0.01).In treated group there were 27 and 7cases given 2 and more than 2 mg of nitroglycerine a week,respectively,and more than 2mg of nitroglycerine a week,respectively,and in control group there were 10and 24 cases.Both groups differed remarkebly( P <0.01).In treated group the drop times of ST cector by the end of periods of 3 and 6 months were(73±15)and(51±15),respectively,and in control group the drop times were(101±23) and (118±24),respectively.The difference between two groups was great significant( P <0.05).The rehespitalized patients were 2 and 7 in treated group and in control groups( P <0.05).Conclusion: As a supportive drug against depression,mirtazapine is very effective for senile unstable angina pectoris combined with depression.
