侧壁螺丝刀在下颌骨骨折坚强内固定的应用

Application of parietal screwdriver in rigid internal fixation for mandibular fracture

目的:探索应用侧壁螺丝刀在下颌骨骨折中进行口内进路坚强内固定的手术方法。方法:对37例下颌骨骨折患者(不含髁状突骨折)采用口内进路暴露下颌骨骨折线,在恢复咬合关系并行颌间结扎后,借助侧壁螺丝刀用小型钛夹板作骨折坚强内固定,术后早期开口训练。结果:37例患者术后开口度明显改善,咬合关系和骨折愈合均满意。2例切口延期愈合,3例存在咬合干扰,4例发生咀嚼无力,经处理后均痊愈。结论:侧壁螺丝刀在采用口内进路坚强内固定治疗下颌骨骨折(不含髁状突骨折)中具有广泛的应用前景。

Objective To explore the procedure of rigid internal fixation ( RI F ) for mandibular fracture with parietal screwdriver via oris. Methods Thirty seven subjects with mandibular fracture ( not including condylar fracture ) unde rwent open reduction via oris, and parietal screwdriver was introduced in the pr ocedure of RIF using minor titanium splint. Postoperative intermaxillary fixatio n maintained a short period, then all subjects performed mouth opening exercised . Results Mouth opening, occlusion recovery and fracture healing were satisfied. Though there were two cases experienced delayed wound healing, three occlusion disturbance, and four masticatory atony, all of the cases got good outcome after management. Conclusion Parietal screwdriver can facilitate the performance of R IF for mandibular fracture via oris.
