吻合血管的阔筋膜瓣移植修复复杂性多指套脱伤

Repair of complicated multiple digit degloving injury with vascularized fascia lata flap

目的探讨采用吻合血管的阔筋膜瓣移植修复复杂性多指套脱伤的疗效。方法应用吻合血管的阔筋膜瓣治疗多指套脱伤3例,筋膜瓣面积最大16cm×32cm,最小3cm×8cm。结果3例筋膜瓣均顺利成活,植于其上的中厚皮片除1例有小块坏死外,其余全部成活,供区有1例部分皮肤坏死。术后随访1～2年,手掌及手背创面均修复,并最大限度地保留了套脱的手指,皮瓣不臃肿,免除二次修整。结论吻合血管的阔筋膜瓣移植是修复复杂性多指套脱伤的有效方法。

Objective To discuss the treatment of complicated multiple digit degloving injury with vascularized fascia lata flap.Methods We repaired 3 cases of digit degloving injury with vascularized fascia lata flap,the largest size of fascial flap was 16cm×32cm,while the smallest was 3cm×8cm.Results The transplanted fascial flap survived in all cases,except for the minor necrosis in 1 cases,the split thickness skin graft survived completely,defects on palm and dorsum of hand were repaired successfully. And it maintained the function of degloving finger furthest. Conclusion It is an effective method to repair complicated multiple digit degloving injury with vascularized fascia lata flap.
