摘要
目的探讨短波胃电刺激对糖尿病胃轻瘫的治疗作用。方法雄性Wistar大鼠30只,随机分为对照组、糖尿病组和胃电起搏组,分别予空白溶剂(对照组)和链脲霉素(糖尿病组和胃电刺激组)腹腔注射,同时于各组大鼠胃浆膜面埋设两对电极,于给药前1周和给药后第4周记录大鼠胃肌电活动,6周后采用酚红排空方法检测各组大鼠胃液体排空,同时取胃幽门区组织作nNOS神经染色,胃电起搏组于检测同时通过胃近端电极给予短波胃电刺激。结果①链脲霉素可诱导大鼠糖尿病,表现为血糖显著升高和体重显著下降,并导致胃幽门区nNOS阳性神经减少;②糖尿病大鼠正常胃肌电活动百分率和胃排空均显著低于对照组;③短波胃电刺激可加速糖尿病大鼠胃排空。结论糖尿病大鼠存在胃运动异常,表现为正常胃肌电活动减少和胃排空延迟,短波胃电刺激可改善该异常,可能是治疗胃运动障碍性疾病的一种新方法。
Objective This study was to observe the effect of short pulse gastric electrical stimulation on diabetic gastroparesis. Methods Ten control rats and twenty STZ-induced diabetic rats were used in this study. Gastric slow waves were recorded at baseline and 4 weeks after the injection of STZ/vehicle. Gastric emptying with or without short pulse gastric electrical stimulation was measured 6 weeks after the injection in a group of 10 diabetic rats each. Results ①STZ injection resulted in hyperglycemia, weight loss and nNOS-positive neuron loss. ②Gastric motility was impaired in the diabetic rats. The percentage of normal slow waves was significantly decreased 4 weeks after the STZ injection. Gastric emptying in the diabetic rats was significantly delayed 6 weeks after the STZ injection. ③Short pulse gastric electrical stimulation accelerated gastric emptying in the diabetic rats. Conclusion The impaired gastric slow waves and gastric emptying are existed in diabetic rats. Short pulse gastric electrical stimulation normalizing delayed gastric emptying in diabetic rats provides a therapeutic strategy of gastrointestinal dysmotility and gastric dysrhythmias may not be the only factor impairing the gastric emptying.
