脑卒中住院患者心理障碍的临床研究

Clinical Investigation of Psychologic Obstacle on Stroke Inpatient

目的 探讨如何及时对患者心理特点作出临床判断,并及时给予心理矫正及对脑卒中治疗预后的影响。方法 160例脑卒中住院患者采用Hopkin’s症状清单进行心理问题调查,根据评定结果,分为心理问题组(A组)与心理活动正常组(B组)两组均对入院时及30天时采用ADL评定、欧洲神经功能缺损评分量表(ESS)及病残程度分级,对两组的恢复转归进行比较。结果 58%的患者有不同程度的心理障碍,且心理问题为多样性,与患者的预后有明显相关性。结论 Hopkin’s症状清单是对脑卒中后心理障碍的一个可靠的辅助诊断手段,具有较好的可操作性和医疗指导意义。并且有效的心理治疗,对脑卒中的预后有明显改善意义。

Objective To probe how to do clinical judgement on time on psychological characteristics of patients with stroke, to observe the influence on prognosis of stroke, and give appropriate psychological treatment. Methods Adopting Hopkin’s test, psychological problems of 160 patients with stroke were investigated. According the evaluation results, all patients were divided into two groups, patients in group A with psychological obstacle and in group A with normal mental activity. On admission and 30 days after admission, with ADL and ESS, the rehabilitations of two groups were compared. Results The incidence of psychological obstacle is 58%. The appearances of problem are varied. The relation with prognosis is obvious. Conclusions Hopkin’s testing is a credible assistant measure for diagnosis of psychological obstacle after stroke. Psychological treatment can improve the prognosis of stroke.
