摘要
目的:探讨高胆红素血症对新生儿听神经及脑干功能的影响,早期评估其预后。方法:根据血清胆红素水平(UCB)把200例新生儿分成A组(128例,256耳,UCB≤342μmol/L)和B组(72例,144耳,UCB>342μmol/L),两组进行脑干听觉诱发电位(BAEP)检测。结果:A、B两组患儿分别有66耳(26%)和92耳(64%)出现V波反应阈升高,两组相比,差异有显著意义(P<0.01)。与A组比较,B组患儿BAEP异常率显著增高(P<0.01)。BAEP异常表现为I、V波分化不良或波形消失,I、V波潜伏期延长,I-V波间期延长。UCB恢复正常后3个月复查BAEP,恢复正常者A组为85.7%,B组为44.7%,组间比较差异显著(P<0.01)。结论:高胆红素血症可导致新生儿听神经和脑干功能损害,血清间接UCB与BAEP异常程度及预后有直接关系。
Obiective:To explore the in fluence of hyperbilirubinemia on the auditory nerve and brainstem function in neonates.Methods:BAEP was performed on 200 neonates(400 ears)divided into Group A(UCB≤342μmol/L,128 neonates,256 ears)and Group B(UCB>342 μmol/L,72 neonates,144 ears).Results: The threshold of wave was increased in 66 ears of Group A and in 92 ears of Group B,and the difference was significant as compared with eath other( P <0.01).Compared with Group A,the abnormality rates of wave I and V,as well as IPL of wave I-V were significantly higher in Group B( P <0 01).Three months after bilirubin concentration normal BAEP normality rate was 85 7% in Group A and 44 7% in Group B( P <0 01).Conclusion:The hyperbilirubinemia neonates could be damaged in peripheral auditory nerve and brainstem function.Both changes of BAEP and the prognosis of hyperbilirubinemia might be associated directly with the serum UCB concentration.
