Effect of early intervention on infant development and the serum levels of growth hormone and insulin-like growth factor I.

摘要 【目的】 研究出生后即开始的早期干预对婴幼儿生长发育的影响及其影响机制。【方法】 将186例刚出生的婴儿分为干预组和对照组,干预组在专业医师指导下采用在家庭与医院进行特殊训练的方法,持续3个月,观察抬头、有意识发出微笑时间,3个月、6个月时身高、体重、心理发育指标变化及生长激素(gronth hormone,GH)和胰岛素样生长因子(isu lin-like growth factorI,IGF-I)水平。【结果】早期干预组婴儿抬头、有意识发出微笑时间较对照组明显提前,体重和身高大于对照组,心理发育综合评分高于对照组(P均<0.05或0.01)。干预组婴儿3个月时1GF-I水平明显高于对照组(P<0.05),而GH水平与对照组无差别(P>0.05),6个月时GH和1GF-I水平均明显高于对照组(P均<0.01)。【结论】0~3个月早期干预对婴儿早期生长发育有明显促进作用,其可能通过影响生长激素和IGF-I水平变化而起作用。 【Objective】To explore the effect of early intervention beginning after birth on infant development and its possible mechanism. 186 infants were enrolled in the study. Ninety three normal infants 2 days after birth in our hospital were assigned to early intervention group, and early intervention were conducted by professional doctors and continued for three months. Ninety three normal infants who had the same enrollment standard as the intervention group were assigned to control group without any special intervention. All infants in two groups were assessed by BaoXiulan’s Developmental Criteria, and height and weight were also measured at the age of three and six months. The levels of growth hormone(GH) and insulin-like growth factor I (IGF-I) were detected simutanously. The infants in intervention group could tilt up head and begin to smile earlier than those infants in control group. Improvement of height and weight in early intervention group were all higher than those of control group( P <0.05 or 0.01); The total development score of intervention group was higher than that of control group( P <0.05 or 0.01); at three months of age , serum level of IGF-1 of intervention group was higer than that of control group( P <0.05 or 0.01), but GH level showed no difference( P >0.05). At six months of age, the serum levels was of GH and IGF-I of intervention group were higher than that of control group( P <0.01). [Conclusion] Early intervention after birth could promote the infant development, height and weight. The hormones GH and IGF-I may play an important role in the mechanism of improvement by early intervention beginning after birth.
