摘要
目的探讨颈椎侧块螺钉技术在颈椎骨折脱位中的应用。方法11例颈椎外伤病人中使用Axis颈椎侧块钢板3例,Cerevifix系统3例,AO颈椎重建钢板2例。术中使用RoyCamille技术置入螺钉8例,Magerl技术置入侧块螺钉3例。结果11例病人经5~22个月,平均19.73个月的临床随访,未发现内固定松动脱落,7例病人获骨性融合。脊髓功能恢复按Frankel分级,5例术前为A级患者均无明显恢复,其余6例术前为B、C级病人的脊髓功能障碍均得到了不同程度的恢复。结论颈椎侧块钢板螺钉系统对颈椎的骨折脱位关节突绞锁具有确实复位作用,并且在进行后路减压的同时能获得稳定的内固定。正确的选择进钉点和入钉方向可以降低神经根和椎动脉损伤的发生率。
Objective To study treatment fracture and dislocation of cervical spine by lateral mass screw technique.Methods Among these patients 3 cases applying Axis system,3 cases applying cerevifix system,2 cases applying AO plate of cervical spine.Among the 11 cases,8 patients were through the use of Roy-Camille technique and 3 patients were through the Magerl technique.Results clinical follow-up was raning from 5 months to 22 months with the mean time 19.73 months.7 cases patients had bone fusion.6 cases patients who was Frankel B,C grade before operation had improvement.Conclusion The lateral mass screw technique can reduction for fractue and dislocation of cervical spine.Implanting screw by correct means can reduce the rate of neurologic complications.
