微循环检测方法在老年下肢动脉硬化闭塞症的临床应用 被引量:7

摘要 目的:探讨激光多普勒血流成像(LDPI)、甲襞微循环综合定量法、经皮氧分压测定三种微循环检测方法在老年下肢外周动脉硬化闭塞症(PAOD)患者的临床应用价值.方法:对48例老年下肢PAOD患者于治疗前及前列腺素脂微球载体制剂(LipoPGE1)治疗后应用三种微循环方法进行检测.结果:治疗后患者临床症状显著改善,有效率85%,与之对应,治疗后甲襞微循环总积分值明显降低(P<0.05),足背激光多普勒血流成像血流值增加(P<0.05),经皮氧分压值增加(P<0.05),三种微循环指标的改善与临床症状改善有很好的平行关系.结论:三种微循环检测方法对于老年下肢PAOD患者的疗效判断具有良好的临床应用价值. AIM: To investigate the practical value of three different microcirculation examination techniques (the nailfold microcirculation, laser Doppler perfusion imaging and transcutaneous oxygen pressure) in aged patients with peripheral arteriosclerotic occlusive disease (PAOD). METHODS: Forty-eight male aged PAOD patients, selected from the hospitalized patients in PLA General Hospital, were treated with Lipo-PGE1. Before and after the treatment, the changes of clinical symptoms, ankle/brachia index (ABI), the nailfold microcirculation, laser Doppler perfusion imaging (LDPI) and the transcutaneous oxygen pressure (TcPO 2) of dorsum pedis were observed. RESULTS: After treatment, the clinical symptoms of the patients were alleviated greatly (effective rate=85%). Significant differences of the three microcirculatory indexes were found before and after treatment. LDPI and transcutaneous oxygen pressure of dorsum pedis were improved significantly (P<0.05). The nailfold microcirculation total scores decreased significantly (P<0.05). CONCLUSION: The three techniques of microcirculation examination are of good practical value in assessing clinical effects in aged patients with PAOD.
