游离丙戊酸血清药物浓度的测定 被引量:6

摘要 目的研究临床患者丙戊酸蛋白结合率。方法用超滤法分离血清中游离丙戊酸,然后用FPIA测定血清丙戊酸及超滤液中丙戊酸浓度,最后计算丙戊酸蛋白结合率。结果丙戊酸蛋白结合率达88.78%-95.91%,平均为93.63%,丙戊酸的蛋白结合率个体差异不大,RSD为2.06%。结论癫痫患者有高的丙戊酸蛋白结合率,临床上不能仅以丙戊酸血清药物浓度为依据,必须综合考虑,建议在适当情况下监测游离丙戊酸血药浓度。 OBJECTIVE: To study protein binding of valproic acid in epileptic patients serum. METHODS: Free valproic acid was extracted from the serum of 30 epileptic patients by ultrafiltration. Then the total level and free level of valproic acid in 30 epileptic patients were determined by fluorescence polarization immunoassay, respectively. Finally serum protein binding ratio of valproic acid in epileptic patients was calculated. RESULTS: Serum protein binding ratio of valproic acid in 30 patients ranged from 88.78% to 95.91%, and the average ratio was 93.63%. Individual variability of serum protein binding ratio of valproic acid in 30 patients was not obvious(RSD = 2.06%). CONCLUSION: A high serum protein binding of valproic acid was found in patients. The level of valproic acid will be comprehensively considered in clinic. It is suggested that the blood drug level of free valproic acid should be monitored in suitable occassion.
