活动性脑出血CT诊断的临床意义

The Clinical Significance of CT Imaging Diagnosis for Active Cerebral Hematoma

目的:探讨活动性脑出血的CT表现及临床意义,指导临床制定治疗方案。方法:回顾2003年9月～2004年12月活动性脑出血(高血压性脑出血20例,外伤性脑血肿12例)病例共32例,分析其CT及临床表现,总结CT及临床表现与临床预后的关系。结果:首次CT检查脑血肿总量≥40mL者19例紧急手术行血肿引流,6例复查CT血肿总量≥40mL者,其中4例即行手术治疗;其余7例给予止血药物保守治疗。CT影像活动性脑出血部分由低密度转变为高密度。30例治愈,但部分遗留不同程度后遗症表现,2例死亡。结论:及时发现和诊断活动性脑出血对临床治疗具有重要指导意义。

Objective:To discuss the CT findings and the clinical significance of active cerebral hematoma for treatment planning. Methods: The CT and clinical manifestation of 32 cases of active cerebral hematoma (20 cases of hypertensive hematoma and 12cases of traumatic hematoma)since September,2003 to December,2004 were retrorespetively analysed.The relations between the CT ,clinical manifestation and the clinical prognosis was summarized.Results: There were 19 cases with the total blood volume ≥40ml were found for first CT scan ,and then these patients were all subjected to blood drainage operation. 6 cases with the total blood volume ≥40mL were found for second CT scan,then the 4 of them were operated immediately.All other 7 cases were given conservative treatment by using antihemorrhagic drugs.The lesion attenuation on CT imaging was changed from hypotensity to hypertensity.30 of all patients were cured,but remained some signs of sequelae.2 patients died.Conclusin: Instant fingding and diagnosing active cerebral hematoma has an important clinical significance.
