鸸鹋油对大鼠浅Ⅱ度烧伤创面愈合中组织修复的影响 被引量:5

Effects of emu oil on tissue repair in wound healing of deep second degree burn in rats

摘要 目的:应用聚维酮碘和鸸鹋油治疗大鼠浅Ⅱ度烧伤,并比较两种药物对创面愈合过程中组织修复的影响。方法:实验于2003-01/04在南方医科大学南方医院完成。108只Wistar大鼠随机分为单纯烧伤组、聚维酮碘治疗组和鸸鹋油治疗组,每组36只。实验时将脱毛区置于70℃恒温水浴中15s,制成10%体表面积浅Ⅱ度烫伤模型(经病理切片证实,本文统称为烧伤)。各组创面分别用生理盐水纱布、聚维酮碘纱布、鸸鹋油纱布覆盖,无菌纱布包扎固定后放回笼中分笼饲养,换药1次/d。观察创面愈合率及愈合时间。各组动物分别于伤后1,3,5,10,14d取创面组织,常规苏木精-伊红染色,观察病理组织学变化;免疫组织化学链霉素-卵白素-生物素-过氧化物酶复合物法检测创面组织增殖细胞核抗原的表达,以标记指数法测定增殖细胞核抗原阳性细胞数,以评估组织创面的修复情况。结果:108只大鼠全部进入结果分析。①创面愈合时间:鸸鹋油治疗组比单纯烧伤组、聚维酮碘治疗组明显缩短[(12.67±0.82),(15.33±0.82),(14.00±0.89)d,F=15.00,P<0.01]。②创面愈合率:鸸鹋油治疗组比单纯烧伤组、聚维酮碘治疗组明显升高[第7天:(73.94±4.46)%,(20.30±3.85)%,(30.71±4.90)%;第10天:(88.45±4.13)%,(40.10±3.14)%,(63.45±4.21)%;第14天:(98.93±1.68)%,(78.67±3.99)%,(90.41±3.09)%;F=443.96,P<0.01]。③病理切片显示鸸鹋油能减轻烧伤创面早期的炎症反应,组织学分析显示鸸鹋油可促进创面的再上皮化和表皮各层的分化。④增殖细胞核抗原在创面组织标本中的表达量:鸸鹋油治疗组比单纯烧伤组、聚维酮碘治疗组明显升高[第5天:(81.83±4.57)%,(32.50±3.08)%,(34.33±2.58)%;第10天:(78.00±3.00)%,(19.16±3.71)%,(21.83±4.53)%;第14天:(65.00±8.29)%,(11.16±2.63)%,(15.00±2.78)%;F=962.40,P<0.01]。随时间推移明显下降(F=95.63,P<0.01)。结论:鸸鹋油能加速大鼠烧伤创面组织修复细胞的增殖,加快创面再上皮化,促进创面愈合。 AIM:To compare the outcomes of tissue repair in deep second degree burn in ra ts treated with emu oil versus povidoneiodine. METHODS:The experiment was conducted in the Nanfang Hospital,Southern Medical University from January to April 2003.Totally 108 Wistar rats were randomly and equally divided into simple burn group,povidone iodine group and emu oil group. The depilatory area was immersed into warm bath at constant temperature of 70 ℃ for 15 seconds,and then the model of superficial second degree burn on 10% o f total body surface area was established(confirmed with pathological sections,b urn was called by a joint name in this paper).After burn,all the wounds were cov ered by normal saline gauze,povidone iodine gauze,and emu oil gauze,respectively .Then all the rats were bound by axenic gauze,back to feed in their separate cag es,and the medicine was changed once daily.The healing rate and time were observ ed in all the groups.The wound tissues were taken at days 1,3,5,10 and 14 after burn.The pathomorphological changes were observed by using hematoxylin and eosin (HE) staining.Immunohistochemical streptavidin- avidin- biotin- peroxidase co mplex was used to test the expression of proliferating cell nuclear antigen(PCNA ).The PCNA positive cells were measured by labeling index to evaluate the tissue repair in wound healing. RESULTS:All the 108 rats were included in the result analysis.① The time for wound healing in the emu oil group[(12.67± 0.82)days] was significantly shorte r than that in the simple burn group[(15.33± 0.82)days] and povidone iodine gro up[(14.00± 0.89)days](F=15.00,P< 0.01).② The wound healing rate was significan tly higher in the emu oil group than in the simple burn group and povidone iodin e group[(73.94± 4.46)% ,(20.30± 3.85)% ,0.71± 4.90)% at 7 days after burn ,(88.45± 4.13)% ,(40.10± 3.14)% ,(63.45± 4.21)% at 10 days,(98.93± 1.68) % ,(78.67± 3.99)% ,(90.41± 3.09)% at 14 days respectively,F=443.96,P< 0.01 ].③ It is shown by the analysis of pathological sections that the early inflamm atory reaction was relieved in the burnt wound by using eum oil,and shown by pat hological analysis that eum oil enhanced wound re- epithelization and different iation of epidermal layers.④ The expression of PCNA in wound tissue specimens in the eum oil group was significantly higher than that in the simple burn group and the povidone iodine group[(81.83± 4.57)% ,(32.50± 3.08)% ,(34.33 ± 2.58)% at 5 days after burn,(78.00± 3.00)% ,(19.16± 3.71)% ,(21.83± 4 .53)% at 10 days,(65.00± 8.29)% ,(11.16± 2.63)% ,(15.00± 2.78)% at 14 d ays,F=962.40,P< 0.01],however it was decreased over time(F=95.63,P CONCLUSION:Emu oil can speed the proliferation of repair cells in wound tissu es of rats,hasten re- epithelization of wound,and accelerate wound healing.
