经阴道三维超声诊断子宫腔内病变 被引量:6

摘要 目的探讨经阴道三维超声在诊断子宫腔内病变的作用。方法对56例疑有宫腔内病变的患者进行三维成像,并与其宫腔镜检病理结果及二维超声结果对比分析。结果56例患者三维成像诊断内膜息肉25例、粘膜下肌瘤14例、子宫内膜增厚13例、子宫内膜癌2例、宫腔积血2例。与病理结果比较,三维超声诊断准确性为92.9%,二维超声为71.4%,两者差异有显著性(P<0.05)。经阴道三维超声对粘膜下肌瘤诊断的特异性、敏感性均为100%,对内膜息肉诊断的特异性、敏感性分别是92.3%、96.7%。结论三维超声对子宫腔内病变诊断的准确性,尤其对粘膜下肌瘤和内膜息肉诊断的特异性和敏感性均优于二维超声。 Objective To evaluate three-dimensional transvaginal sonography(3D-TVS) in diagnosis of uterine cavity lesion. Methods 3D-TVS were performed in 56 patients with uterine cavity lesion, the results of which were compared with that of pathology and hysteroscopy, and two-dimensional transvaginal sonography(2D-TVS). Results Of 56 patients, 25 suffered from endometrial polyp, 14 from submucous myoma, 13 from endometrial hyperplasia, 2 from endometrial cancer, and 2 from coagulated blood clot in uterine cavity. The accuracy of diagnosis by 3D-TVS was higher than that by 2D-TVS. The specificity and sensitivity of 3D-TVS in diagnosis of endometrial hyperplasia was 92.3%、96.7% respectively, while 100% and 100% in submucous myomis. Conclusion 3D-TVS in diagnosis of uterine cavity lesion is superior to 2D-TVS.
